Demonstrates effective academic communication and appropriate referencing using the APA7 style. 2 CASE STUDY SCENARIO Background This case study introduces you to Aleki Tupuola, a 42yo man married to Donna for the last 15 years. Born in Australia to Samoan parents, Aleki was raised in Brisbane amongst his large, churchgoing family. A heavy drinker in his youth, Aleki settled down once he was married, and lives on the Sunshine Coast where he has worked as a housepainter. Aleki and Donna have three children – son Jayden (14yo), daughter Sophie (10yo) and their ‘surprise baby’, son Fetu (18 months old). Aleki’s mother is now in her early 70s, and his father died three years ago from complications of diabetes. Aleki is one of six children, all still living in and around SE Queensland. Donna’s parents live in Townsville, and she is very close to her sister Kaylee who lives in Rockhampton. Diagnosis and prognosis A year ago, Aleki presented to hospital with constant, severe ache over his right upper quadrant, unrelated to movement, with occasional sharp spasms of epigastric pain. He had persistent nausea, loss of appetite, and weight loss. He believed he had a duodenal ulcer, as two of his brothers also had them. Upon further investigation, Aleki was found to have Stage III exocrine pancreatic cancer. He underwent a Whipple’s procedure followed later by chemotherapy. He has been unable to work since and although he is paid a pension, the financial strain has been great, as Donna only has part time work whilst looking after Fetu at home. With mild but persistent pain and low-grade nausea over the months that followed, further investigation showed that Aleki’s cancer had spread to his liver, now designated as Stage IV. Aleki and Donna were told by his oncologist last week that he has a prognosis of ‘perhaps three months’. He and Donna have told their extended families, and the two older children. Donna has been appointed his Enduring Power of Attorney for health matters and, with their GP’s help, completed an Advance Care Plan. Presentation Aleki has been admitted to the oncology unit with rapid exacerbation of pain, nausea and yellowed sclera. His abdomen is a little swollen. He is very weak, with very poor sleep at night; he is saying that his father ‘died like this’. Donna is visibly distressed when she visits Aleki and looks exhausted. Jayden and Sophie have visited a couple of times too, but Fetu has not seen his father in hospital. Question One
Aleki has been presented with the stage IV Pancre
atic cancer and there are minimum possibilities of having long life survival rates at this stage except a few exceptional cases. As per previous studies conducted over the patients such as Aleki, the survival rate at Stage IV is 1% or even less than 1%. However, it should be noted here that the prognosis of stage IV refers to the deviation in the health condition of the patient, which has been one of the prominent reasons of Aleki’s admitting to the hospital (Cooper et al., 2018).
He is taking up the regular course of medication in spite of the pressure on behalf of the family that he should be sent back to home. This is one of the major reasons behind the increasing/decreasing ratio of five-year life expectancy of Aleki as he has been predicted to have a maximum of three months’ time as of now. What makes his condition worse is the family history of pancreatic cancer, which unfortunately did not go through formal diagnosis and his father died in the same way as he is showing symptoms (Shahid et al., 2018).
Additionally, the five-year life expectancy ratio can be understood by the following graphical projection:
As per the observation of the graph above, one can notice the differences between the early and the late diagnosis of pancreatic cancer. Under categories such as 1A and 1B, the expectancy ratio decreases as the cancer grows in stages. Under stage IV, there are only a few chances of survival left for the patients suffering from pancreatic cancer.
In case of Aleki, it is mentioned in the case study that the cancer has now spread to the liver, which makes his condition even more vulnerable than any other patient of pancreatic cancer does on the same stage. The projected life expectancy in the graph above also verifies the predication made to Aleki because he is having three to five months of time of survival.
Question Two
As per the observation of Aleki’s pain, there are specific tools, which can be applied to assess the degree of pain, which he is having with regard to structure his medication. There can be categorical pain assessment, which can also be applied to his case namely; the types of pain here include mild, moderate and severe. The consistency of pain also plays pivotal role in this process of evaluation (Cooper et al., 2018).
Aleki has so far complained of having mild pain, which is consistent in the upper right quadrant. Here, the noticeable aspect of his pain is that he is having occasional sharp spasms of pain, which subside as the time passes. Nevertheless, the mild pain continues to affect his working. Thus, on categorical grounds, his pain is mild and consistent with sharpness at a frequency of recurring mode.
Furthermore, there are some other tools to examine the degree of pain, which Aleki is having at present and this should include the post chemotherapy life, after he was diagnosed with stage III pancreatic cancer. Verbal Rating Scales (VRS) and face Pain Scales- Revised (FPS-R) are the two methods, through which his pain can be evaluated for further medication.
Verbal Rating Scales is the questionnaire-based tool, in which formal examination of the pain includes asking open-ended questions with a view to know the physical issues that might affect the movement or non-movement based activities of the patient. Moreover, these questions relate to the category, intensity and frequency of pain, which Aleki is going through. At the same time, the application of FPS-R pertains to record the facial expressions, which change according to the pain conditions of the patient.
For example, when sharp spasms of pain would hit, Aleki’s facial expressions would change and he would clearly reflect that he is suffering from acute pain for a while. FPS-R is also used to recognise the frequency and lasting nature of the pain in patients (Aoun et al., 2020). For Aleki, the consistency of pain is something that needs formal management and medication to control it.
