The aim of this written assessment item is to apply the nursing process in providing family centred care. When an infant, young child or adolescent experiences a health or social issue, the issue can impact upon all family members. Nurses working in acute care and community settings need to understand the functioning of the family unit so they can care for and assist the whole family.


Instructions:


This 2,000 word written assignment has two distinct parts that you should address separately. Please use headings for each part.


You do not need to provide an introduction or a conclusion for the written assignment or any of the parts.


Use the SUPPLIED scenario for your entire assignment.
Part 1 - Nursing Care of the Family: Assessment (500 words)



Create a genogram to visually depict the family’s structure. You must use the PowerPoint slide which will be supplied to you within the Assessment Folder on Learning@Griffith course site to create your genogram. Save the slide as a picture file (*.jpeg), and insert the picture into your document.















  • Below the genogram, summarise the structure of the family to demonstrate your understanding of the family assessment findings.





  • Use the Australian Family Strengths Nursing Assessment Guide (AFSNAG) to identify and briefly describe two (2) strengths of the family you are assessing.



















Part 2 - Nursing Care of the Family: Planning, Implementing and Evaluating


(1,500 words)






  • Select two (2) issues/challenges for the family or a member of the family you have selected. These issues may be identified by the nurse, family or both. These can be health, social, or developmental family issues/challenges e.g., breastfeeding, social isolation, transition to parenting; they should not be ‘medical’ issues e.g., diabetes, high blood pressure.





  • For each issue/challenge identified in the family assessment (allow approximately 750 words per issue):








  1. a) Describe the issue




  2. b) Plan nursing care




  3. c) Implement nursing care




  4. d) Evaluate nursing care














o Use appropriate evidence from scholarly literature to describe the issue and



discuss what is known about the issue/challenge.



o Provide a relevant nursing goal and justify the goal (explain why it is relevant to the issue) using appropriate evidence or policies.


o Outline one nursing intervention that supports the family to achieve the goal. Each nursing intervention should be supplemented by the recommendation of an existing online resource for the family and an appropriate referral.


o Describe how you would evaluate the effectiveness of the intervention to address whether it met the planned goal of care.

Family:


Family Scenario: The Kaufman Family




  • ● Tina Kaufman, 15 year old female, in year 10 at high school




  • ● Barbara, 48 years old, Tina’s mother, is a homemaker.




  • ● Stephen, 45 years old, Tina’s step-father. He owns his own construction business


    Background:







  • Tina lives with her mother and step-father in a 4 bedroom house.





  • Tina does not see her biological father since her parents divorced in 2006.





  • Stephen and Barbara were married in 2008.





  • Tina loves singing, dancing and acting in school plays. She also enjoys playing school soccer


    and is in the representative soccer team. She gets good grades is school but she has to work hard to keep up with her schoolwork and activities.


    Extended family:







  • Tina’s biological father, Mark aged 43 years old, lives close to Tina but he has not maintained a relationship with her since the divorce.





  • Mark has remarried Simone, who is 33 years old.





  • Tina also has a brother, George who is 23 years. He lives interstate and Tina has no


    relationship or contact with him either. He lives with his partner, Jeremy. Tina’s parents do


    not speak to George.





  • Barbara’s parents Birgit (aged 72) and Reinhard (aged 76) are both alive and still living


    independently. Barbara tries to visit them every fortnight but this has been putting some strain


    on her relationship with Stephen, as he does not get along well with them.





  • Stephen’s father, Christopher died 2 years ago when he was 70 from lung cancer. Stephen’s mother, Robyn, aged 68 years old has dementia and now resides in a nursing home. Stephen


    does not like to visit her as she no longer recognises him.


    Current situation:







  • Most of the family's interest revolves around Tina’s athletic activities such as baseball and church related activities such as Sunday services and youth centered groups. The family attends a full gospel, non-denominational church on a regular basis. The family is of German descent and likes to keep to themselves. They rarely see anyone on a personal basis and have no close personal friends. Tina’s recreational habits consist of playing softball and basketball and attending youth oriented activities at church such as youth camp and bible studies.





  • Tina usually goes to the library to study during the lunch break instead of the cafeteria to eat lunch. She says she doesn’t need to eat if she is not hungry. She prefers grabbing a snack in the school vending machines or at a convenience store on her way home after school like most of her friends do. Tina’s mother Barbara, says Tina is so busy that she has no time to eat meals with the family.





  • Lately, Tina’s friends have made comments to her about how much she has grown and “filled out.” Tina is concerned that she is gaining too much weight. Tina weighs herself every day and is 56.5 kilograms. When her weight goes above 56.5 kilograms, Tina limits her food intake to bagels, low-fat yogurt, fruits, and vegetables (mainly salads) and does not eat meat products and desserts. Barbara is not sure whether to support Tina’s efforts to lose weight.

Tina is due for a sports physical assessment, before making an appointment for the physical examination with the family doctor. Barbara decides to bring Tina to see you, the School nurse to assess Tina and share her concerns about Tina’s eating behaviours.
