Rationale: The purpose of this assessment task is to assess the student nurses ability to think critically and apply the clinical reasoning cycle to plan patient care. As a Registered Nurse, you will be required to make judgments and prioritise your patients care multiple times per shift. This assessment task is designed to develop the sophisticated thinking abilities that are essential to prioritising, planning and providing safe and effective patient centred, nursing care. Importance of Nursing Problem
Nursing plays an important role in patient care and it becomes more significant in the post-operative situation. The nursing problem that is significant in the scenario of Darren Roberts 50-year-old male who has just had an Open Cholecystectomy. The patient has a history of hypertension, hyperlipidemia, and GORD. After the surgery, the saturated oxygen level was less than 95% and the respiratory rate was 10Rpm. Nursing intervention at this stage is an important context. Cholecystectomy is a procedure in which the gallbladder is removed and after the surgery, it is effective to be under professional nursing guidelines. The post-operative intervention by the nurses is a sign of effective nursing care and monitor the patients.
Particular Risk for Darren Roberts
The particular risk that Darren Robert is facing following his surgery is respiratory depression which has a connection to anesthesia and opioid medications. It is evident from his respiratory rate which is less than 10 Rpm and his saturated level of oxygen was less than 95% on RA with shallow breathing.
