You are required to create an XML document to record a doctor’s patient visit list.

The XML document will be used for a doctor’s surgery to track patient details and visits and the status of their accounts and allow for iterative (repeat) development. Use the information tabled above to create a valid and well-formed XML document.
Use the column names to create the outline for your XML document. The information in columns date and status are attributes of the element visit and are required. All elements can appear more than once in the document.
Use XSLT to transform your document into an XHTML document that displays the above information so that it can be recognized by a browser.


Complete the following activities:


1.Write a report on the project’s requirements.Review the case study information above.
Write a short (about a page) report that documents the following:•The purpose, expectations and functionality of the XML document;•How you have used the software engineering life cycle (SDLC) process to develop the document.•Code program. This should follow the appropriate standards and declaring DTD, required entities, elements and their attributes. Draw a tree structure diagram detailing your plan for the design and build of the XML document.•The hardware, software and tools needed to design the document.

2.Create the XML document.Create your XML document including an internal DTD subset which defines all required entities, elements and their attributes.Create the document following appropriate standards for a well-formed and valid XML document.
Take a screen shot of the document and save it as XML Document.


3.Create an.xsl document to output XHTML information to the browser.Take a screen shot of the file. Save this as XHTML File.


4.Create a CSS style sheet to style the XML document in the browser.Take a screen shot of the file. Save this as CSS File.


5.Test the document.Test the document offline against given specifications and confirm your document’s validity.
Test the document online against specifications, using an XML validator (http://www.w3schools.com/xml/xml_validator.asp) and confirm the document’s validity.
Take screenshots of the validation results. Save these as Validation Screen Shots.


6.Send an email to your assessor.The text of the email should be in grammatically correct English, written in an appropriate (polite, business-like) style.It should introduce and summarise the contents of the attachments.The email text should give a short (a few sentences) report on the document’s validity.
