Assessment Task To achieve a satisfactory result This unit is a competency based unit and students will be marked against the following criteria: Not competent (NC)Competent (AC) All individual assessment tasks are graded as Satisfactory (S) or Not satisfactory (NS) and the student needs to achieve a grade of Satisfactory in each assessment task to achieve a Competency grade in the Unit of Competency. Assessment Task Assessment Task 1_ – Academic Report – Two persons 1. You are required to select two (2) persons with a chronic health issue. These persons will be based on people you select from your Clinical Placement. Examples of health issues could include but are not limited to: · Chronic Obstructive Pulmonary Disorder (COPD) · Heart Failure (Congestive Cardiac Failure – CCF) · Stroke · Diabetes Mellitus (DM) · Chronic back pain · Hepatitis C · Renal Failure · Osteoarthritis (OA) · Parkinson’s Disease (PD) · Cancer (Ca) · Multiple Sclerosis (MS) · Dementia · Angina · Sarcoidosis · Chronic Fatigue Syndrome · Chronic skin conditions · Incontinence · Long Term Unconscious person · Motor Neurone Disease (MND) · Epilepsy Research the Chronic Health Condition/s by accessing relevant, recent articles and evidence based practice literature. Write two (2) separate reports that address the points below: Section 1: a. Provide a ‘holistic’ history the person. b. Identify the clinical manifestations of the chronic health problem and possible impact on body systems. c. Confirm and summarise the person’s understanding of their chronic condition. d. Describe the pathophysiological processes associated with the person’s chronic illness. You are encouraged to support your written response with diagrams and/or photographs. e. Identify any co-existing health problems associated with the person’s chronic condition and describe the related pathophysiology. f. Identify three specific nursing interventions that are relevant for the person and their chronic health problem and state how and when the intervention is implemented, why the intervention is implemented and expected outcomes related to the intervention. g. Discuss the potential and/or actual social, emotional and psychological symptoms that impact on the person’s ability to achieve maximum performance in ADLs. You may also need to consider the impact on the person / family / significant others. h. Write a Nursing Care Plan for the person using the nursing process. A Nursing Care Plan template has been provided. This Care Plan should include nursing diagnosis of identified problems, and evaluation of the relevant nursing interventions for the person in consultation with the person and interdisciplinary team. Take into consideration the person’s self-management, rehabilitation, restorative and palliative care needs. i. Conduct an academic search of literature and identify current best practice associated with one nursing intervention for the person’s chronic health problem. j. Describe three strategies that actively involve the person to self-manage their chronic disease. k. Identify other supporting agencies / health professionals that are currently contributing to the care of the person’s individual needs. l. Identify the level and type of contribution made by the person’s family and other carers. m. List three resources and support services which could be accessed by the person and their significant other(s) on discharge.
