Introduction
    Clinical reasoning is the process by which the nurses and clinicians gathers information about the patient through effective interaction. The given case study represents an 85-year-old woman, Mrs. Yvonne Stewart, who was taken to the NSW rural hospital for left hip replacement. Several safety measures were taken by the nurse Riley and Alex which improves our knowledge on clinical reasoning. Clinical reasoning include several factors like gathering information about the patient, processing them and setting the treatment goas accordingly to get the best outcome for the patient. I am obliged to get a chance to explore and gather skills on clinical reasoning and patient safety which will help me in my future practice as a nurse. We will explore the practical application of the clinical reasoning with the help of the clinical reasoning cycle in the case of Mrs. Stewart.
The Patient’s Solution
The healthcare environment is highly susceptible to spread the infections that comes from the several types of patients and practices. As mentioned by Gan, Lim and Koh (2020), the patient’s safety includes effective communication, avoiding infection, and reducing the errors in treatment to get the best possible outcome for the patient. In the case study, Mrs. Stewart and her husband was communicated about the surgical procedure. Riley conveyed the anaesthetic process and provided privacy to Mrs. Steward to change into the Surgical gown. As presented by Kumar, Dubey and Ranjan (2019), anxiety is common in the case of surgical patients. They undergo stress before the procedure which can be reduced by reassurance with effective communication. The student nurse Alex was advised to continuously communicate with the patient to increase her confident and maintain the five rules of hand washing before handling Mr. Stewart. These will maintain the safety of the patient in the hospital environment and result in a positive outcome. Full set of observations including the vitals were advised to be collected by nurse Riley which will gather knowledge and help in the clinical decision-making for Mrs. Stewart.
