This semester project is written from the collaborative perspectives and efforts of an interprofessional team comprised of Chief Nursing Officer (CNO), a Chief Nursing Informatics Officer (CNIO), the Health Information Technology (HIT) Department, and the Privacy and Security Department in a large healthcare system. The interprofessional team requests the creation of an Excel spreadsheet and a written report based on information retrieved from an EHR used in ambulatory care for patients with hypertension.As this is a simulation, you will not have data to study if the change improves or does not improve. Make the assumption that the change improves the process, but not at the expected threshold. This assumption will guide the Act Phase.



Project 1 – Completed as an Individual (Excel spreadsheets)


· Using the files extracted from OpenMRS, the interprofessional team requests the information for patients with hypertension to be entered into an Excel spreadsheet for analysis.


· Using an Excel Spreadsheet (Label the Spreadsheet Tab Hypertension)


· For each patient and each patient visit enter into the Excel spreadsheet for men and women. Create a spreadsheet for men with hypertension, a spreadsheet for women with hypertension, spreadsheet for all patients with hypertension

Write a report based on the data analysis and results completed in Excel.Use a cover page.The written report should follow APA style. References should follow APA style. Be sure to use headings to organize the content. The Excel spreadsheets should be included in the report. Do not submit as separate files. Under paragraph set before and after to 0 to ensure exact double spacing. Correctly label the tables and figures using APA style


o Introduction –Include one paragraph the reader to hypertension, one paragraph to OpenMRS. Include a rationale/purpose for the project. Inform the reader how the report is organized (what should the reader expect to find in the report).

What is the definition of adult? What is the definition of child? Were both adults and children included in the analysis (what defines an adult, what defines a child, need references)? What is the definition of hypertension for an adult? What is the definition of hypertension for a child?


· Results - will be presented in the Results section of the written report.


o Results should briefly (1 paragraph) introduce the reader to the results. The reader should be informed in the text of the tables and figures. Refer to APA regarding how to label a figure and how to label a table.


o Results should be presented in Tables and Figures.


· Discussion of results


o How are the results similar or different from what is found in the published literature? How does this project add to the understanding of hypertension?


o Describe process of cleaning the data. What were things you found by observing, reviewing, checking the Excel spreadsheets (this could include discussing missing/not recorded data, data outside the expected range for an attribute, age of the individuals with a particular diagnosis, conversion of Celsius to Fahrenheit, organization of the spreadsheet columns, checking for inaccuracy in data entry, including children in the analysis.


· Limitations of the current EHR


· Recommendations


o A process improvement study of workflow in measuring blood pressure and recording in the EHR. The process improvement is about the process of measuring blood pressure using an evidence based-guideline to check blood pressure. The process improvement change isNOTabout patient/staff satisfaction, white coat syndrome, or patient teaching. If the data are inaccurate these factors will not improve a flawed process.

o What are the key points the reader should remember about the project? Do not introduce any new content.

· Follows APA style, uses headings to organize the content, margins set at 1 inch, exact double spacing, correctly labels figures and tables following APA style, spelling, grammar, punctuation. Exact double spacing is achieved by selecting paragraph from the ribbon in Word and setting before and after to 0. This will ensure exact double spacing.
