Introduction
The primary rationale for providing the professional portfolio was to learn from my experience and reflect on how things proceed while I was handling the case of Meredith Green who was a 20-year-old female with a history of depression and bipolar disorder. These twenty days of Workplace Experience (WPE) placements was an am
azing learning and enriching experience at Princes of Wales Hospitals in depression and bipolar disorder (Prince of Wales Hospital., 2019). Therefore, by virtue of this report I present to you my learning journey in form of reflections applying three-layered conceptual map that offered critical key issues, important stakeholders as well as detailed scenario in the placement. These experiences challenged me as well as helped me apply the concepts I learnt in class in real life. Essentially, I have applied three-layered conceptual maps that highlights key issues, all the stakeholders that are relevant in the case as well as record the details of the scenario while performing the placement (Anderson, C., 2018). Therefore, I have tried my best to explain the patient-centred process as well as registered nurse (RN) which is standard for practice. The sheer amount of information about the clinical situation as well as the exposure I received is commendable. Perhaps, this experiential learning will always help me to distinguish myself from the crowd. I have applied Gibb's Reflective Cycle which was developed by Gibbs in the year 1998 that provides me necessary tools and framework to reflect on the feeling, describe my experience, evaluate, analyse, conclude and offer action plan.
Reflection
I was assigned the case of 20-year-old female with a history of depression and bipolar disorder. She had traumatic incidence in her past and was reported to have attention deficit hyperactivity disorder. While working on her case and deeply comprehending the case files I have understood that she had suicidal tendency with deliberate self-harm. For instance, inserting harmful things into her rectum, numerous incidences of aggression as well as compulsive self-harm on the ward (Berman, A., Snyder, S. J., Levett-Jones, T., Burton, P. & Harvey, N., 2017).
While getting the vitals of Miss Meredith Green I found that there is alteration in the reports received. There were many signs of hypertension and anxiety attacks which wasn’t mentioned in the findings. Furthermore, her heart rate used to increase which could be analysed by the respiratory rate escalation. Furthermore, the blood pressure during aggressive incidence went as high as 180 systolic and 99 diastolic pressure. Meredith Green's observation of respiratory rate (RR), blood pressure (BP) and Spo2 were all reflecting signs of problem and marked yellow so that they can be reviewed later. This review was also marked on the standard adult general observation (SAGO) chart. Therefore, I immediately connected with the supervisor and informed him about my observations and concerns regarding the case. I didn't stop there as I knew I need to communicate these findings to all the concerned stakeholders and therefore I also informed the attending nurse. Meredith have often complained of feeling numb and depressed while I was examining her. (Cortez, S., Dietrich, M. S., Wells, N. & Cortez, S., 2016).
