EMS responds to the home of a known cocaine abuser. On arrival, the patient is found to be a 21-year-old man, lying unresponsive in a closet. He has a pair of panty hose tied around his right upper arm. There are obvious “tracks” on both arms. The patient’s roommate says the patient had been high on cocaine and suddenly became unconscious. Further assessment reveals: Level of consciousness: Unconscious; responds to painful stimuli by occasional tonic and clonic jerking movements and incoherent speech. Respirations: 28/min, shallow. Pulse: 124 beats/min, irregular. Blood pressure: 190/102. Skin: Warm and moist. Pupils: Dilated, slow to react. Medical control instructs you to start an IV of normal saline at a keep-open rate.
