1. We need to evaluate the major signs of the patients. We will check the blood pressure of the patient along with the respiratory rate, the heart rate, body temperature and the level of oxygen saturation. These check-ups can be conducted by the nurse in the emergency rooms of the hospital.
2. We can also give the patients oxygen through the nasal cannula 2LPM. This will help the patient to breath smoothly. The patient is suffering from chest tightening and also can impact the oxygen supply in the body of the patient. The care must be conduced by the nurse.
3. Medications are necessary by the patient in order to relieve him from chest pain. During the emergency situation the patient can be provided with nitro-glycerine. Therefore, the patient might have a heart attack if the chest pain he is experiencing is not cured.
4. The other symptoms the patient is suffering from can be managed. We have to evaluate other factors also which might be contributed to the main complaint of the patient. We must also check the blood sugar level of the patient and can must also check if it has correlations with the conditions. Therefore, by recognizing the underlying conditions will gradually help to manage the patients.
5. The patient must be provided a safe and comfortable environment where the room temperature must commensurate as per his health condition. The environment must be peaceful which will help the patient to recover quickly. It will also trigger less discomfort. Further, the patients who are suffering from pains must be kept in a stress-free environment which will help them to recover quickly.

Immediate treatment actions: We have been able to immediately address the symptoms which the patient is suffering from. The patient is suffering from acute chest pain. Proper medications need to be given to relieve the patient from chest pain. Further, he must be provided with ancillary treatment for recovering him from the chest pain. Therefore, the patient needs nitro-glycerine and proper oxygen supply for getting relieved from the chest pain. This problem must be addressed immediately. Further, the patients must be evaluated to check if the patient is suffering from any underlying diseases which might deteriorate the patient’s health situation.
Referrals: Referrals to reputed hospital or doctors can be done if the patient is not relived even after providing the above therapy. Here the doctor can request for a laboratory examination for getting confirmed and investigate on it further. The physician can also request the patient for imaging modalities and look for any associated causes. Finally, if nothing works the patient can be referred to another healthcare provider who is reputed, renowned, have many years of experience as a cardiologist and have the knowledge to successfully cure the patient.
Longer-term actions: There are certain risks factors which are being observed in the patients. These are type 2 diabetic, high rate of cigarette smoking and unhealthy eating habits and diet. These cases must be addressed first as this will help to lessen the probability to reduce the cases such as stroke. The patient must be advised for a healthy lifestyle by consuming a healthy and balanced diet which will consist of more vegetable and fruits and less amount of carbohydrates, fats and sodium. Since the patient is suffering from type 2 diabetic, he must be suggested for intake of oral hypoglycemics agents which will help to reduce the blood sugar level. The patient must be advised to stop smoking of cigarettes. We can refer the patient to a support group who can help him to stop smoking cigarettes. The nurses can also discuss the ill effects of smoking cigarettes with the patients so that due to fear the patient will stop smoking. This will definitely help the patient to recover.
