For each group of student names, choose 1 of the 2 Tasks. Use the raw data in the data table and provided key components to create a Step in the Task Sequence and a data statement (O), comparative data statement and additional analysis (A) and a appropriate plan (P). Use the SOAP format on the last page for your assignment. An extra credit opportunity of up to 5 points is optional. Instructions: 1). Use the correct format for a SOAP note based on examples and class discussion. 2). Use strong clinical writing, with correct grammar and spelling. Up to .5 point (from the total score) may be deducted for errors to mechanics or grammar. 3). Create a client name and an age that might be reasonable. 4). Create one “S” based on what might be appropriate for the client’s age or function given the information in provided data. This is at your discretion. 5). Write and include a TS Step that aligns with the task you choose. You have been provided 2 sets of data which reflect the client's performance today (4/13) and last session (4/11). 6). Create an ”O” which documents client performance for today’s data (4/13), as indicated. 7). Create an “A” which will include comparison to the previous session’s performance (4/11) and any relevant analysis of today’s performance. For each set of assigned data, you will need to write an accurate “A” statement to reflect if the performance has: ● increased, or increased with criteria met; advance task ● decreased, or decreased with criteria met; advance task ● remained consistent; (consider if criteria was met to advance task) ● is compared to (todays session is the FIRST session a more advanced tep) 8).
