Collecting and Analyzing Qualitative Data Root Cause Analysis BUS665 – Managing Business Processes 1 Agenda ANALYZE Study (getting at the root cause) Fishbone diagrams 5 Whys Force field analysis Brainstorm solutions FMEA (also could be performed in the DESIGN – test ideas phase) 2 Fishbone Diagram What is it? Also called “cause and effect” or “Ishikawa” diagram, it is used to… Organize potential causes of a problem (called the effect), usually because there are so many potential causes of a given problem – push BEYOND symptoms to uncover ROOT causes Select the most probable case of the problem (effect) Verify the cause and effect relationship between the problem (effect) and the most probable cause(s) Why use one? People are frequently overwhelmed by the number of causes related to a problem, a cause and effect diagram helps a team identify the various causes and narrow them down to the most probable cause. Categories of causes, or the “fishbones” might include 6Ms… Mother nature (environment) Machines Materials Methods Manpower (people) Measurement OR – the clusters from an affinity diagram 3 Fishbone Diagram Why is Joe still single? 4 Fishbone Diagram 5 Why do we miss deadlines in our organization? Fishbone Diagrams Why do we experience delays in custom orders? 6 5-Whys What is it? Method for pushing people to think about root causes; requires you to as “why” five times Prevents a team from being satisfied with superficial solutions that won’t fix the problem in the long run How to create one… Select any cause (choose from a fishbone diagram or the tallest bar on your Pareto chart); make sure everyone has an understanding of what it means Ask “why does this outcome occur?” Select one of the reasons for “why” in #2 and ask “why does that occur?” Select one of the reasons for “why” in #3 and ask “why does that occur?” Continue until you feel you have reached a potential root cause (nothing sacred about 5) Example: “Why are we late in delivery?...because the copier jams... “Why does the copier jam?”…because of high humidity in the copier room… “Why does high humidity cause jams?”…because the paper absorbs moisture and sticks together. If you can’t do anything about the paper absorbing moisture, look to solving the problem of high humidity in the copier room 7 5-Whys Problem: client is refusing to pay for leaflets printed for him… 8 5-Whys Problem: coolant leaking from a machine… 9 5-Whys Problem: A resident had a bad fall in their room. 10 Forcefield Analysis What is it? Decision-making tool that identifies the forces driving toward an outcome or goal and the forces that are blocking or driving away from the outcome; each are quantified to determine if favorable forces (acting for) outweigh unfavorable forces (acting against) How to create one… Define the change you want to see (goal, vision, desired future state) Brainstorm the forces that are favorable to change; brainstorm the forces that are unfavorable to the change Evaluate the driving and restraining forces on a scale of 1 (weak) to 5 (strong) and total each side (can do without numbers, as well) Review – Is there a favorable balance? What forces have flexibility to be strengthened (driving forces) or weakened (restraining forces)? Create an action plan **Is inherently subjective, should be used with other decision-making tools 11 Forcefield Analysis 12 Quantify each… Forcefield Analysis 13 Forcefield Analysis 14 FMEA What is it? Stands for – Failure, Mode, and Effects Analysis Helps to… Identify the ways in which a product, service, or process can fail Estimate the risks associated with specific failure causes Prioritize actions to reduce risk of failure Types Design: analyzes a new process, product, or service before rollout to understand how it could fail (exposes potential safety hazards, malfunctions, defects, customer dissatisfaction, etc.) Process: Used to improve existing transactional and operational processes to understand how people, materials, equipment, methods, and environment cause problems System: Analyzes systems and subsystems in the early stages of concept and design 15 FMEA How to create one… Review the product, service, or process (if process, start with steps that contribute the most value) Brainstorm then sort possible failure modes (ways in which the component, sub-assembly, product, input, or process could fail to perform its intended function) List one or more potential effects for each failure mode - if the failure occurs, what are the consequences? Assign ratings for severity (1-10, 10 = most severe) and occurrence (1-10, 10 = most likely to occur) List current monitoring and controls for each failure then assign a detection rating to each (1-10, 10 = least likely to be noticed given current control methods) Calculate risk priority number (RPN) for each effect by multiplying the three numbers (severity * occurrence * detection) Use the RPNs to select high-priority failure modes (highest RPN attended to first; any failure mode with a severity rating of 10 must be worked on first regardless of RPN, due to potential impact on customers) Plan to reduce or eliminate the risk of high-priority failure modes (brainstorm potential solutions, develop a plan of action, assign individuals) Carry out the plans and document actions taken Recompute RPNs 16 FMEA 17 FMEA 18 FMEA 19 Thank You 20 BUS-665-Managing Business Processes Guidance for Discussion Board Posts using PowerPoint Templates Several of the discussion posts in this course ask you to create a PowerPoint template to support your response to the discussion post, which is then accompanied by a description of that template in the provided text box. To ensure your success on these posts, below is some guidance on how to approach these assignments. · Always begin these types of discussion posts with the PowerPoint template. These are meant to be tools to help you think through the material in the case; think of them as resources for helping you to outline, think critically, and process the content of the case and how it relates to the course material. · The information in these templates should be simultaneously concise and specific. These have been designed for the purposes of demonstrating you have not only read the case in its entirety, but have also taken the time to process the information and connect it back to the assigned lecture material. These should demonstrate that you know the content of both well enough that you can concisely summarize examples and demonstrate thoroughness and mastery of the concepts. · You can adapt the template in any way to meet your needs. However, you need to stick to the one slide limit; please see the prior bullet point about demonstrating meaningful understanding through specific and concise responses. Also, feel free to adjust the size of the font and/or that of the table/”Smart Art” graphic, as well as add additional bullet points, etc., to accommodate the material you want to include. Make sure all of your content fits within the boundaries of the slide (when in “presenting” mode, anything that falls outside the boundary of the slide does not show up). · These will have more text than if you were to actually do a PowerPoint presentation. These are a learning and understanding aid; you will not actually be presenting the material. This means you can feel comfortable about including more text than you might otherwise include for a presentation. However, make sure they have a professional appearance, as you are still submitting these for credit in a graduate course (see bullet point above about making sure the content fits within the boundaries of the slide). · A good approach might be to use the PowerPoint as a place to take notes on the case, making note of anything that seems relevant. Then, once all this information has been captured, reviewing what is in the slide and eliminating weaker points, as well as making sure points are specific and concise. · Your written post should be completed after the PowerPoint template is complete. The written posts are meant to summarize the visual, highlight important points, and in some instances, make the connection to your professional work. PowerPoint Presentation PROBLEM: WHY? WHY? WHY? Problem: The government withheld the actual amount and paid the amount as per the contract. Therefore, the actual amount for the flyover project was left unpaid. It created financial issues for the management and led to losses in the project.
The project was completed late by a year as there was no provision for escalation in the contract. This was the first mistake made by the manager. Escalation clause would have helped the project to manage increasing cost of raw materials which became a problem during lockdown. Also, apart from this other materials which were required for construction would have been covered and the profit would not have been compromised. Due to the pandemic the company was not able to arrange labor as they migrated to their hometown due to lockdown. The flyover was of importance because two major metropolitan cities...
