In this case study, the doctor was unable to stop Mike from making decisions that seemed
dumb and might harm James. To protect the patient's autonomy, he was following by the law, as well as internal and external (government and institution) laws and his professional code of ethics. Mike acted as the patient's voice because James was a minor. When it came to diagnosing James and recommending the best course of action, the doctor did his research. Mike was obviously feeling things, and he was unaware of the risks associated with his decisions. So, in this instance, I think the doctor should have gone to the ethics board to see if there was anything else that could be done.
James's doctor is in the best position to advise on his medical care, but spiritual support must be acknowledged as a viable alternative to surgical procedures.
