INTRODUCTION
There are many patients, particularly the older patient, and those patients who do not have control over themselves because of the surgery or any severe illness are the one who are much prone to fall. This further increases the risk among the patient of further getting severely affected and it can also lead to the death of the patient. Hence, it is important to keep the patient
at the safe position to avoid any kind of fall. In this report a discussion is done on how the fall of the patient is avoided in different departments of the hospital where the patients are admitted, by making necessary arrangements. Also, the discussion is done on the 3-small step PDCA scale, which will help in proper planning of healthcare intervention in order to avoid any type of fall.
PART 1
One of the major safety indicators in healthcare sector is patientโ€™s fall. This is one of the major concerns because of patient falling down from the bed or chairs because of the lack of proper care or support is leading to the loss of life of many people. As discussed by Kiyoshi-Teo et al. (2019) that safety and quality care provided to the patient in the hospitals of Australia is of utmost importance for all the patients, and the major focus is given on the Australian government and the service provider. Therefore, it is very important to continuously monitor all the safety procedures in order to ensure that all the safety measures are followed in order to prevent any fall of the patient. In the general medical ward of the hospital, there are certain steeps which needed to be followed in order to prevent the fall of the patient. At first the identification of the high risk patient is necessary, because that will help in taking steps such as putting railing on the side of the beds, or if the patient is not stable then a safety companion must be placed at all the time in order to have a watch over the patient (Tajali et al., 2017). A bed alarm must be put beside the bed in order to enable the patient to call for help or need whenever the patient feel like. And lastly, it is important to continue with visits at regular intervals to have a close observation of the patientโ€™s safety and also ensure any need is addressed immediately (Mota de Sousa et al., 2017).

Falls among the patient during the hospitalization or in their home have become a major contributor of the injuries, and sometimes it also results into the death of the patient. As discussed by Matchar et al. (2017) that apart from death those who get injured due to fall results into the prolonged stay in the hospital and also it imposes a significant burden over the patient and their family with respect to economic and social burden. This also increases burden over the healthcare organizations in order because of the prolonged stay of the patient and also it creates problem for the population at large. Therefore, it is necessary to decrease the number of falls and also injuries among the patient, which requires the modification of the protocols related to current fall prevention with the help of evidence-based practices (Uchmanowicz et al., 2018).
The literature review purpose is to identify the studies and the guidelines of fall prevention, which are of high-quality content and also, they significantly demonstrate the reduction in fall among the patient. The hospital authorities require to prepare a fall prevention program which will include all the guidelines and protocols that is required to be followed by the healthcare professionals in order to minimize the fall among the patient. Further added by Ganz and Latham (2020) that evaluating the effectiveness of the fall prevention program design and implementing the strategies of risk reduction which will help in improving the safety of the patient and it will also decrease the falls and the injuries related to the falls.
