Health History


Mrs. J. is a 63-year-old married woman who has a history of hypertension, chronic heart failure, and chronic obstructive pulmonary disease (COPD). Despite requiring 2L of oxygen/nasal cannula at home during activity, she continues to smoke two packs of cigarettes a day and has done so for 40 years. Three days ago, she had sudden onset of flu-like symptoms including fever, productive cough, nausea, and malaise. Over the past 3 days, she has been unable to perform ADLs and has required assistance in walking short distances. She has not taken her antihypertensive medications or medications to control her heart failure for 3 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure and acute exacerbation of COPD.



Subjective Data



  1. Is very anxious and asks whether she is going to die.

  2. Denies pain but says she feels like she cannot get enough air.

  3. Says her heart feels like it is "running away."

  4. Reports that she is exhausted and cannot eat or drink by herself.



Objective Data



  1. Height 175 cm; Weight 95.5kg.

  2. Vital signs: T 37.6C, HR 118 and irregular, RR 34, BP 90/58.

  3. Cardiovascular: Distant S1, S2, S3 present; PMI at sixth ICS and faint: all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac monitoring indicates a ventricular rate of 132 and atrial fibrillation.

  4. Respiratory: Pulmonary crackles; decreased breath sounds right lower lobe; coughing frothy blood-tinged sputum; SpO2 82%.

  5. Gastrointestinal: BS present: hepatomegaly 4cm below costal margin.

Mrs. J is affected to a fever which is measured to be at 37.6̊ C. This fever is of low grade. Also the heart rate of Mrs. J is gauged to be at slightly above the normal rate at 118 bpm and is considered to be irregular. The
normal rate is at 60 to 100 bpm. The rate of respiration of the lady is measured to be at RR 34 while the normal rate is considered to be at 12 to 30 bpm. The normal blood pressure level of a person is generally between 120/80 but for Mrs. J it is measured to be at 90/80 which is very low. She also suffers from various other diseases like cough with blood, problems in liver like hepatomegaly, tinged sputum and also suffers from pulmonary cracks. The oxygen saturation level of Mrs. J’s blood is at 82% Spo2 and it is much below the standard level of 94% to 99%. This signifies that Mrs. J is also suffering from pulmonary diseases. At the beginning and during the end of systolic pressure Mrs. J is also having resonances.

The medicines prescribed to Mrs. J are appropriate as per her present situation. She is having some difficulties due to her pulse rate being very high than the normal and due to the pulmonary cracks she suffers from breathing problems.
Enalapril (Vasotec) - Mrs. J is suffering from low blood pressure and hence this medicine is essential to elevate her blood pressure level. This drug enhances the angiotensin II, makes the arteries of the patient thin and increases the blood pressure level.
IV morphine sulphate (Morphine) – This medicine helps Mrs. J to cure her from the pain and depression she is suffering from.
IV furosemide (Lasix) – This medicine was used to cure the hepatomegaly and pulmonary disease of Mrs. J and restrict from getting affected to heart attack (Francis, 2017). It is also used for controlling depression, breathing problems and in treating the edema of fluid retention problem in Mrs. J.
Metoprolol (Lopressor) – This helps to reduce blood pressure, hypertension and prevents the patient from getting strokes, kidney failure, reducing pain in the chest and faster recovery post a heart attack (Francis, 2017).
Oxygen delivered at 2L/ NC – This provides a supplementary oxygen boost to the patient with acute respiratory problems.
Inhaled short-acting bronchodilator (ProAir HFA) – This medicine helps Mrs. J from shortness of breathe which is caused generally due to her chronic pulmonary disease. This drug works on the muscles of the patient thus preventing the breathing problems (Francis, 2017).
