  1. Jessica is a 14-year-old client who recently graduated from a small 8thgrade class of 22 students. She started 9thgrade at a large high school two days ago and after staying at the school for 15 minutes, had to go to the nurse because of stomach pains, nausea, and severe anxiety. She was referred to you for a psychiatric evaluation. During the evaluation, you discover that she had separation anxiety disorder when she started kindergarten years ago. This was helped by providing a transitional object, meeting her very supportive teacher prior to starting school, having a tour of the school and classroom, and having the teacher introduce her to another student prior to starting and having a few play dates with this new friend as well as allowing the parents to walk her into the class. Now, 8 years later, the same diagnosis has resurfaced. Jessica refuses to go to school because she is fearful that the symptoms will return and she will be embarrassed and ostracized by the other students. You institute psychotherapy but decide to prescribe medication as well because the therapy may take several weeks to allow her to return to school and you know school refusal is a psychiatric emergency. What would you prescribe and why?
