1. After reading the United 173 mini case (see below attachment to read the case), what are the main reasons that the cockpit crew allowed the aircraft to run out of fuel? (answer based on the unit of analysis – individual, group, and organizational levels) 
The reason the crew let the cockpit run out of fuel were more than one. Based on unit of analysis, these can be classified as follows-
Individually- The crew got otherwise engaged in observing and trying to diagnose the problem that arose with unusual indications, the very reason landing was aborted. While this was a prudent decision, the crew didn’t pay enough attention to their fuel state which was being used at a much rapid pace due to the fault in the plane.
As a group- The crew failed to engage in successful management, coordination and teamwork with regards to the failure of aircraft systems. The captain, as the leader, was unable to monitor or respond to the fuel shortage in a timely manner resulting in fuel exhaustion due to preoccupancy with the gear malfunction.
On the organizational level- The crew was not properly trained regarding fuel quantity assessment measures. So, the responsibility of being careless falls on the entire organization and not...
