Miguel, a 26-year-old male, presents at your offi ce for a routine semi-annual dental hygiene appointment. Miguel’s medical history is unremarkable and he appears to be in good health. He takes no medications, but as you review his medical history with him, he mentions a current and recurring problem with red and irritated eyes for which he says he has an appointment with an eye doctor next week. He also had a urinary tract infection a week ago. His dental history has not changed in 6 months. You perform the head, neck and intraoral exam, noting both the red irritated eyes and the lesion pictured in Figure 12.42. He describes the 2-cm ulceration on his tongue as very painful and he has continued to traumatize the lateral border of the tongue because of previous ulcer-like lesions and scarring of the tissues.
