    The topic is hand hygiene monitoring and it is an important element of hand hygiene campaigns. It is important to promote the safety culture in the hospital and reduce the errors. If hand hygiene is not properly monitored than it would have adverse effects. It would
risk the health of many patients; they would be at risk of illness or even death. The nursing practice issue related to hand hygiene monitoring is skin irritation from hand washing agents, understaffing, high intensity of patient care, lack of materials, and the cultural factors due to which there is poor compliance with the hand hygiene compliance.

    I have selected the hand hygiene monitoring topic because hand hygiene is the most important factor. It helps in the prevention of infections associated with health care. Poor hand hygiene a threat to the safety of the patients. If hand hygiene is not monitored it can cause morbidity and mortality globally. The cost of healthcare associate infections is a lot, to deal with healthcare associate infection, the healthcare workers must comply with the hand hygiene compliance.

    The poor compliance of healthcare workers related to hand hygiene guidelines is due to insufficient number os sinks, belief that the use of glove will avoid infection, low risk of acquiring infections from the patients, sinks are located inconveniently, and ignorance and disagreement with the guidelines and protocols. In the formulation of PICOT questions, the population is health care workers and patients, intervention is proper handwashing and personal protective equipment, the comparison is no proper hand washing and use of personal protective equipment, the outcome is decrease spread of health associate infections, time is during hospitalization. PICOT question: Does the monitoring of hand hygiene reduce the risk of spread of health associate infection for patients and healthcare workers?

    From the systematic review and the strength of evidence, it is important to monitor hand hygiene. Monitoring hand hygiene will save a lot of costs and it will also reduce the risk of infections. The hand hygiene monitoring must start with creating awareness among the patients and health care ware. Training and education must be provided to explain to them the importance of hand hygiene. The healthcare workers and patients must comply with the guidelines of hand hygiene. The barriers in complying with the guidance of hand hygiene must be addressed. Some of these barriers are skin irritation caused by hand hygiene agents, wearing gloves, and lack of active participation in promotion at the institutional level as well as individual level.

    Non-compliance with the hand hygiene recommendation is due to failure in removing the gloves after patient contact. Between patient contact, washing and reusing gloves is ineffective. In a study that involved artificial contamination, it was observed organisms were culture from 4 percent to 100 percent of the gloves.
