a. Chronic asthma
Symptoms include shortness of breath, panting and heaviness in chest and in lungs.
b. Chronic obstructive pulmonary disease
Symptoms are pain in the left side of chest, blurring of vision, giddiness and shortness of breath.
c. Chronic pain
Symptoms range from—but are not limited to—problems in movement, particular organ based issue as well swelling in the region where the pain is happening.
d. Chronic renal failure
Problems in urination, small stones in kidney and pain in the left or right side of lower body
e. Congestive heart failure
It involves pain in chest, which is severe, shortness of breath and frequent strokes of unconsciousness.
f. Diabetes insipidus
Frequent urination leads to dehydration of body and results in possible seizures.
g. Diabetes mellitus
Enlarged pancreas, increased blood sugar level as well as problems with blood pressure control are symptoms.
h. Eczema
Inflammation, redness or swelling at skin leads to this condition, which is also itchy at sometimes.
i. Bladder incontinence
Slow urine passages, less stream of urine causes this condition.
j. Motor neuron disease
Issus with muscle movements of hands and legs or numbness of body is a general symptom.
k. Severe traumatic brain injuries(neurological injury)
Symptoms include palsy strokes, lack of focus, frequent giddiness and lack of movements of body parts perfectly.
l. Long term unconsciousness or coma
This is often referred as a condition of being inactive body state due to the inability of brain functioning well.
m. Osteoarthritis
Pain in joints of hands and legs, less fluidic joints or pain in the movement of the body parts like hand or leg are common signs.
n. Parkinson’s disease
It is basically related to the weakness of nerve cells in brain and results in lack of memory and focus.
o. Rheumatoid arthritis
Weak immunity when healthy cells in body are attacked by antibodies. Symptoms include pain, blurred vision etc.
p. Middle cerebral artery(MCA)stroke
Giddiness and weak focus are the two major symptoms along with pain in any side of upper body.
q. Systemic lupus erythematosus (SLE)
Redness and swelling of skin, weakness, shortness of breath and weakness in function are major signs.
r. Chronic back pain
Problems in sitting or walking as well as consistent stiffness of back are major signs.

These are focus points in order to address the chronic disease issues and the way they can be prevented from having long-term care, in case they are controllable through initial measures.
c. Innovative Care for Chronic Conditions Model
This is the innovation of treatment models and application of the new care intervention, which are innovated, and they are useful in the process of chronic care provided to individuals.
d. Elements of the Chronic Care Model
The basic elements are clinical resources and self-management system along with having strong and well-integrated clinical information systems.
e. Purpose of the National Strategic Framework for Chronic Conditions
