Assessment 2 Part Three: Care Plan Review





Overview


It is now 12 months later and the client has attended the preventative, curative and rehabilitative services that you referred them to in Item 5 Part Two. Their health condition/s have improved significantly, however you must now follow-up with the client, and evaluate the interventions.


For this part of the assessment, specify in 150-200 words (in total) how you would review and update their plan of care so their needs are still met. Your response should be tailored to the client you worked with in Part One, and include:


· Noting how you would follow up/evaluate on the issues the client had and the actions they agreed to


· The information you should update on their health plan, in consultation with the client


· The relevant community support services you may need to keep them in contact with


· Two (2) recommendations for the client on how their health plan can be monitored and maintained.
