There are several key environmental forces that have shaped the health information system (HIS) landscape and the ongoing evolution and adoption of systems and technologies. Various factors have shaped the current and future use of health information technology (HIT). Understanding this background allows one to understand the roles of the private sector as well as government efforts to advance HIT. The adoption of electronic health and medical records (EMR/EHR) to replace traditional, paper-based patient records is a primary example of a government-led initiative.


In years past, sharing of information was challenging at best, and often absent. As technology has evolved, so has the provision and organization of healthcare services: stand-alone physician practices have been replaced with large, multi-specialty medical groups; once independent hospitals, pharmacies, imaging centers, and laboratories are now part of larger healthcare systems with services integrated under one organization (Buchbinder & Shanks 2017). Today, various information technologies facilitate the sharing of medical information, and created new avenues to collaborate.
