Discussion & Justification of First Two Nursing Care Priorities using LJ Clinical Reasoning Cycle
According to European Heart Association (2018), using LJ Clinical Reasoning Cycle, the nursing care priorities of Mrs. Deborah may be described in eight steps as follows:
1. Patient Situation
Deborah Scott‘s age is 65 years and she holds multiple risk factors causing repeated falls. She has a history of osteoarthritis and insulin dependent diabetes. These factors may lower down her recovery from fractures. She has fallen thrice in last six months of time. So it may be fear of falling which drives her to fall again. While living with her husband and pet, she may be subjected to isolation. She lives on her own. Being an old age woman also increases her susceptibility for low bone mass in comparison to men.
2. Collection of Information
She is taking multiple medications for different health conditions. Her chest x ray shows broken ribs and chest walls. She has been admitted twice before for the fall. She has a medical history of hypertension, Diabetes II insulin dependent, Osteoarthritis, and chronic COPD. Her lifestyle seems to have no or minimal exercises or social involvement.
3. Process Information
Deborah Scott is old age woman with multiple risk factors leading to falls. According to WHO reports, the people aged 65 years or more are at 28-30% higher risk of falling repeatedly (Viera, 2016). She has a history of osteoarthritis and insulin dependent diabetes. The bone problems like Osteoarthritis become main cause of fractures and fall related injuries after a fall. Osteoarthritis affects 9% of the people greater than age 55 years (Gale, Cooper & Sayer, 2016). Moreover, in women there is 49% greater risk of lower bone mass and increases progressively till the age of 70 years (Gale et al., 2016).She has fallen thrice in last six months of time. So, the people who have fallen recently are more likely to fall again than the people who have no history of falls. While living with her husband and pet, she may be subjected to isolation and loneliness which increases her susceptibility to face unexpected falls. The old age people living on their own are prone to unpredictable falls. After the age of 60 years, people face more functional decline in daily living activities and are at higher risk of recurrent falls (Gale et al., 2016). The fear of falling also reduces functional ability and mobility in aged people while increasing their dependence.
