Case Study


Kathy is a 59-year-old woman who has multiple medical conditions including type 2 diabetes, hypertension and longstanding musculoskeletal pain following a car accident in 2014. Her medical history includes, anaemia, knee replacement, and multiple accident and emergency (A&E) admissions for asthma.




Kathy takes Metformin to manage her diabetes and uses both reliever and preventer inhalers to manage her asthma. Her GP has also recommended lifestyle changes (diet, exercise, lower alcohol consumption etc.) to improve her health. However, Kathy finds it difficult to put these changes into action.




Kathy lives alone in a one-bedroom flat which is in a poor state of repair (damp, mould on the walls, unreliable plumbing etc.) and is located adjacent to a busy main road. She no longer works due to ill-health. She socialises regularly with friends at the local community centre and meets her nephew and niece every weekend to have a coffee and go shopping.

Why might Kathy be feeling this way? Explain the role of the patient (Kathy) and carer (her doctor, nurses and healthcare assistants) with reference to the balance of power.

Task 1 -
· The term “Health”
WHO defined health as “a state of complete physical, mental and social well being and not merely the absence of any disease or affirmity”1.
· Definition of Social model of health, Medical model of health and Holistic model of health
· Social model of health –
The social model of health emerged in era of t
he 1970s, the time when people realised the importance of the different environmental and external factors and their contribution to the different notions of health. The social model of health can be described as a conceptual framework within which good health and wellbeing can be achieved through the process of directing the efforts towards addressing the different determinants of health such as social, economic and environmental factors. The different determinants of health are inclusive of food, shelter, socio-economic status which includes employment, education and so on, culture, access to healthcare, location and social connectedness. From the case of Kathy, we could see that Kathy stays alone in a one bedroom flat which is in a poor state of repair (damp, mould on the walls, unreliable plumbing etc). Kathy’s poor housing and surrounding environment could be a reason for her ill health (social model)
· Medical model of health –
The medical model of health can be defined and described as the approach where one seeks to treat and cure a particular illness and prevent different forms of complications, however does not aim to explicitly put an end to the growth and development of diseases before they arise biologically. That is to say, this model primarily views the body in the form of a machine, in which certain parts will inevitably break down. However, the broken down parts can be fixed in an effective manner through the involvement of diagnosis, treatment by using medicines such as antibiotics and technologies such as surgeries in order to remove or repair a certain part of the body. In the case of Kathy, we could see that Kathy has multiple health conditions, such as Type 2 diabetes, hypertension and musculoskeletal pain. Apart from these, she has other medical history which includes anaemia, knee replacement, multiple accident and emergency admissions for asthma.
· Holistic model of health –
The holistic model of health can be defined and described as the particular way of looking at health in a comprehensive manner so as to include physical, mental, social, emotional as well as spiritual health and wellbeing. When an individual focuses on the holistic model of health, he/she tends to focus on the concept of wellness rather than recovering from any particular form of disease or illness. In the mentioned case of Kathy, we could see that on weekends Kathy meets her niece and nephew and go for shopping and have coffee with them. Along with these, she meets her friends regularly. All these people in Kathy’s life supports her both physically and mentally and thus keeps her mind at peace2.
