Case Study

Kathy is a 59-year-old woman who has multiple medical conditions including type 2 diabetes, hypertension and longstanding musculoskeletal pain following a car accident in 2014. Her medical history includes, anaemia, knee replacement, and multiple accident and emergency (A&E) admissions for asthma.

Kathy takes Metformin to manage her diabetes and uses both reliever and preventer inhalers to manage her asthma. Her GP has also recommended lifestyle changes (diet, exercise, lower alcohol consumption etc.) to improve her health. However, Kathy finds it difficult to put these changes into action.

Kathy lives alone in a one-bedroom flat which is in a poor state of repair (damp, mould on the walls, unreliable plumbing etc.) and is located adjacent to a busy main road. She no longer works due to ill-health. She socialises regularly with friends at the local community centre and meets her nephew and niece every weekend to have a coffee and go shopping.

She has been on the hospital ward for 3 days and as well as experiencing physical symptoms (nausea, shortness of breath, exhaustion) is feeling vulnerable and powerless. Why might Kathy be feeling this way? Explain the role of the patient (Kathy) and carer (her doctor, nurses and healthcare assistants) with reference to the balance of power.
