Week One Reflective Journal


In this first activity you will complete a well-studied assessment tool called the Rand 36 Item Health Survey. The scores will help you take a general look at components of your mental and physical health. To interpret your scores, recognize that lower scores indicate areas where there is opportunity for change and higher scores indicate healthier behaviors and feelings. Now that you have reviewed the material in this week’s module, reflect on how it pertains to your life.



In this week's reflective journal entry, be sure to address each of the following questions:



  1. Refer back to the results of yourRand 36 Item Health Survey. In the areas where you scored high, what helps you to maintain healthy behaviors? Identify factors in each of these Socioecological model areas: individual factors and beliefs, interpersonal (friends/home/work), community settings, and societal (norms/values/sectors).

  2. Identify one specific dimension of wellness that you want to address at this time. Where you are on the Transtheoretical Model (Stages of Change) scale for this area?

  3. Set aSMARTgoal (Specific, Measurable, Achievable, Realistic, & Time frame) for ONE behavior that YOU want to change for betterhealthand that is realistic for you to modify at this time in your life. Remember to choose an Action, not a Result and include a Treat/Reward component. Examples of a SMART goal: I will drink at least 64 ounces of water a day. Each week I am successful I will download a new iTunes song.

  4. Devise anAction Planby writing 3-5 action steps to ensure you successfully accomplish your SMART goal. Include one accountability step. Examples of action steps:


I will carry a 32 ounce refillable bottle of water with me to class/work each day.


I will refill the bottle at lunch time.


I will not carry change for the soda machine so that I am not tempted.


I will set reminders in my cell phone.


I will challenge my friend, Sara, to do it with me.



You will be journaling on your progress each week.



NOTE:This is a gradable journal with no partial credit. Journal entries MUST be a minimum length of250wordsto receive credit.Each week’s Reflective Journal will include specific journal "prompt" questions, which must beanswered specificallyand mustcontain details, examples, and reflectionin order to earn the points for the journal. Why the word count? Because this is a “writing to learn” exercise, where you will explore your thoughts and feelings and work out concerns.




Type andsave your submission in a Word fileand thencopy/pasteinto Blackboard.
Do not attach a file. Students are responsible for checking to make sure that posted journal entries actually display in the journal. Ans) I’m able to maintain healthy behaviors of emotional well-being and social functioning by keeping up optimistic thinking. Self affirmative talks help me to maintain my self esteem. Supportive family and friends gives me confidence to face any problem with positive attitude.
