There is a word limit of 1500 words (500 words for each question). Use your computer to total the number of words used in your assignment. However, do not include the reference list at the end of your assignment in the word count. In-text citations will be included in the additional 10% word count. If you exceed the word limit by more than 10% the marker will stop marking at 1500 words plus 10%.

    Short answer case study
    Mr Jock Jagger, a 67-year-old male has been transferred from the Emergency Department to the Medical ward. His wife brought him in to hospital from home as she no longer felt comfortable managing her husband’s increasing back pain and was worried about malnutrition as Jock had not eaten for two days.
    On admission to the ward the clinical manifestations were:
    • Pain score 6/10
    • Mild restlessness
    • Nausea
    • Dry mouth
    Mr Jagger has a history of bowel cancer and had aggressive treatment 12 years ago. Three (3) months ago, he noticed he was becoming increasingly lethargic, after investigations he was diagnosed with stage 4 (metastatic) bowel cancer and has opted for no further treatment and to focus on managing his symptoms.
    Whilst examining Mr Jagger, you chat about being cared for at home and he explains that he wishes to return home, he agreed to come to hospital because his wife is worried about caring for him as he continues to deteriorate. His Advanced Care Plan does outline his wish to stay home but he does not wish to burden his wife.
    Short answer questions
    1. Outline and explain the delivery of comprehensive care for Mr Jagger who wants to return home. Your answer should include the role of the multidisciplinary team using a palliative approach. (500 words)

Mr Jagger, being a bowel cancer patient, wants to return home and take treatments as per recommendation from the doctors at home. In such case, the wish of the patient must be fulfilled of receiving proper care to control the adverse symptoms of the disease. He must receive a proper, co-ordinated healthcare from the family and the medical team dealing his case. For such comprehensive care, a palliative care is needed by the multidisciplinary team to treat the patient properly at home.
The multidisciplinary team must include expertise from several healthcare professionals to control the symptoms through treatment giving the patient relief until death arrives. As mentioned by Pask et al. (2018), a multidisciplinary team should include the patient, the family, care provider, and the healthcare professionals. All the members, co-ordinate with one another in the process of serving a palliative care to the patient according to his/her wish.
According to Kassianos, Ioannou, Koutsantoni and Charalambous (2018), a proper execution of palliative care by the multidisciplinary team helps the patient to receive a patient-centric treatment, which makes the patient satisfied, and the produce a positive result to the treatment. In this case, Mr Jagger is in his fourth stage of bowel cancer and the palliative care that can be provided to him is the palliative chemo to reduce the symptoms such as pain, nausea, dryness of the mouth, and restlessness.
At home, the family members and the care providers must be alert in taking care of Mr Jagger and act accordingly if any such symptoms arise. The patient must be rushed to the hospital and always stay in touch with the medical professional to control any emergency. A comprehensive care must be based on proper communication and co-ordination across the multidisciplinary team who are involved in taking palliative care of the patient (Australian Commission on Safety and Quality in Healthcare, 2022).
The proper treatment plan must be followed at home by the care provider and the family members, which are provided from the hospitals. The patient co-operation is the main criteria for treatment of such life taking disease. As stated by Hui, Hannon, Zimmermann and Bruera (2018), a proper time management and an active team are the most important factors in producing effective palliative care to the patients.
The proper healthcare plan, which must be followed by the multidisciplinary team, should include giving mental support to the patient and increasing their will to live. It should also include following proper test and diagnosis schedule, providing medicines at the required time and taking quick action during emergency, visiting the hospital periodically for receiving chemotherapy and reducing the unplanned admission of the patient by controlling the symptoms through proper communication with the medical team.
Such procedures if followed properly by the multidisciplinary team, the patient can receive a satisfaction of staying at home in his/her last days, which may increase the possible years of their life. The comprehensive care hence will provide the physical, mental, and cognitive support to Mr Jagger, which will help him to co-ordinate with the treatment plan provided by the multidisciplinary team and produce a positive outcome.
