This task provides an opportunity to explore a specific case study encompassing all the units' learning outcomes, your theoretical learning and application of clinical nursing skills within this unit. This task will assess your understanding of conducting a comprehensive assessment of Boris introduced within the HSNS264 Moodle site (Module 4 Case study 1) who has TB, or Fiona, as per the case study below. You will be required to provide a comprehensive overview of relevant patient care and management- such as an in-depth understanding of the patient's condition, risk factors, related anatomy and physiology, pathophysiology, relevant diagnostics and treatments. Develop a nursing care plan and provide a relevant educational teaching plan prior to discharge from hospital.




Instructions


Explore either the Boris or Fiona case study, and consider a comprehensive assessment of their condition and the relevant patient care, medication treatment and management required. Provide a detailed discussion of related anatomy, physiology and pathophysiology. Discuss the pharmacological management of either condition, including the pharmacodynamics of the medications. Develop a nursing care plan prioritising his care and any relevant patient teaching prior to his discharge from hospital.

Case Study 1



Tuberculosis (TB)


Boris Vasilescu of Romanian heritage is a 62 year old factory worker. He has had an ongoing productive cough for 5 months with intermittent chest pain, experiencing night sweats, extreme tiredness, recently coughing up blood stained phlegm. His wife Magda has reported that he won’t eat the meals she cooks as a result; he has lost 10 kg from when his symptoms first started.


Upon presentation to hospital he is exhibiting the following physical symptoms: sharp chest pain, clammy skin, unilateral crepitation (crackling), shortness of breath, a respiratory rate of 20 breaths/min, temperature 38°C, oxygen saturation is 98% on room air, Pulse 110, BP 130/76, Weight 70kg.


He has had a chest X-ray showing upper lobe and cavity infiltrates suggestive of TB. A few days later - Boris has been diagnosed with active TB. He has been given appropriate antimicrobial medications upon discharge from the hospital.

On follow up , one month after discharge from the hospital his wife Magda discloses that he does not always take his medications, correctly or on time and she was worried about his long term health. Boris states that he started to feel better after a week or two of taking his medications and in his view were no longer necessary.

Introduction
Tuberculosis is the airborne infection, which mainly affects the lungs of the patients, known as pulmonary tuberculosis. Næss, Kirkevold, Hammer, Straand and Wyller (2017) viewed that there are recurring conditions, the situations of revival of the disease, in patients who do not adhere to the provided care plan particularly when, it is left in the middle of the process of cure. The case study of Boris projects severe state of tuberculosis developing over the course of time and he highlighted some primary symptoms of the onset of the disease.
The pathophysiological changes in his condition are subjected to the intakes of prescribed drugs and their effects over the course of two weeks. However, there is a need of helping Boris to be aware of the facts related to the revival of the disease, as per the observation of his behaviour in following the manner of having the drugs prescribed for him. It is significant for him as well because of his age; in elderly people, the chances of revival of tuberculosis are more prominent than any other age range.
Signs, Symptoms and Pathophysiological Changes in Boris’ Condition
For Boris, the inflammatory process of tuberculosis is conventional; it is to understand the inhaling of bacilli initially and its effect on histological level (Bojovic et al., 2018). There are multiple criteria, which need to be understood in the context of finding the type and stage both of the disease for Boris at present. For a patient like him who suffers from M Tuberculosis, staining of the bacilli is important especially when it is acid fast by property.
Boris has initially complained of having chest pain, which is due to the increased infection of acid fact bacilli in his chest. As mentioned by Khanal et al. (2017), as Bacilli replicate locally, without apparent damage to the host, this stage is called symbiosis and macrophages accumulate and divide. The amount of viable bacilli becomes stagnant in stage 3, the stage at which caseous necrosis first occurs, as their growth is hindered by the immune response to bacilli-released tuberculin-like antigens. Step 4 is the process that normally decides when the condition becomes clinically apparent.
In this case, cell-mediated immunity plays a significant role. The immune response of cytotoxic delayed-type hypersensitivity destroys these macrophages, causing an expanded caseous centre and disease progression (Bojovic et al., 2017). Additionally, the bacilli multiply at extracellular level when liquefaction of the caseous core occurs, often reaching very large numbers. A tissue-damaging delayed-type hypersensitivity reaction, which erodes the bronchial wall, forming a cavity, is triggered by the high local concentration of tuberculin-like products extracted from these bacilli (Ravimohan, Kornfeld, Weissman & Bisson, 2018).
Thus, the proliferation and productive stage of tuberculosis is likely to occur in the coming phase to Boris and he definitely needs to understand the process and importance of medication to control it. When he was admitted to the hospital for care, he had shown clear symptoms of infection as the strains of blood were occurring in the spits. The shortness of breath was the symbol of weakening of the lungs, especially the alveoli sacks to retain the air pressure. The opening of the sacks was weak and as per the examination, he was prescribed for having certain medications....
