Catherine Wilson is a 35-year-old woman. She currently lives in a small unit and has been unemployed for the last 7 years. Catherine is known to the local mental health services and to the local drug and alcohol services with a history of substance use disorder and major depression disorder resulting in various admissions to an acute mental health ward. Catherine left school after completing year 12 and went to vocational college to study childcare. During this time, she began to feel low in mood and would not attend college due to sleeping most of the day. She describes not having any energy or desire for anything. Alcohol, cannabis and sleep provided a sense of comfort and refuge from feelings of worthlessness and hopelessness she was experiencing at the time. Catherine was first admitted to an acute ward when she was 20 years old. Since then she has had 4 further admissions. Catherine has been admitted to the acute mental health ward where you work as a mental health nurse. The admission occurred after a neighbour became concerned about Catherine’s apparent deterioration in health and called the mental health crisis and assessment team who on assessment believed Catherine required further assessment and treatment in an inpatient setting. Catherine agreed to the admission. Within the mental state examination, you gather evidence that Catherine is experiencing multiple issues. There is evidence of lowness of mood with blunted affect. Answers are largely monosyllabic though at times she can expand on a few questions being asked. Catherine denies any suicidal ideation and states ‘she doesn’t have the energy to kill herself however people would probably be better off without her’. Catherine confirms a history of cannabis and alcohol use and over the last 2 years she has been using Crystal methamphetamine (ICE) as a way of treating her depression since the antidepressants she had previously been prescribed did not appear to be as effective anymore. As Catherine’s named nurse you are aware of the comorbidity issues apparent in this case and must take this into consideration when developing a care plan that meets Catherine’s needs and enables her recovery.
