We have to
create one template in ExcelSpreadsheetto collect responses to the three
questions outlined in the Word file. The survey is for the call operator to
enter the answers (as coded 1,2,3 for each question) in the data file.










Then a 2ndspreadsheetin the same Excel file would link the data
findings to create bar/pie charts for each question. So that when operators
would make the inputs, the charts would be updated automatically.















Lastly, we must
include some simple instructions on entering the response data being collected.





Call Service Evaluation Surveys    Check In Call Service Recipients [Questions to be asked at the end of the Call with recipient’s permission] 1. How satisfied were you with the information you received today? 1 – Satisfied 2 – Somewhat Satisfied 3 – Not Satisfied 2. How comfortable do you feel following up with the referrals you received today? 1 – Comfortable 2 – Somewhat comfortable 3 – Not comfortable 3. And lastly, how helpful was this call for you? 1 – Helpful 2 – Slightly helpful 3 – Not helpful at all
