    Name of work placement:
    Name of supervisor:
    First name (only) of client/person:
    Age:
Write a summary of this person with details such as age and background (it does not need to be an older person).
Catherine, a 40-year-old single mother of four under the age of 20, struggles to make ends meet. Ovarian cancer has been found in her body. I work as a personal care assistant for the elderly woman's Community Care team, and her family is picky about anyone they hire. They don't know where to turn for more help. Advanced Care plans are the subject of conversation. Her mother succumbed to same cancer that claimed her mother's life. Catherine's age is the same as mine.
What types of support services does this person receive?
As a result of her ovarian cancer diagnosis, Catherine is now getting assistance from various sources. Monitoring, pain management, and other health care assistance.
What is your role in providing support to this person and what are your limitations?
The role is the personal care assistant of Catherine. A lack of clarity in roles may create unnecessary tension for the client and oneself. It may also lead to a lack of competence, misunderstanding, and decreased patient care quality.

Which services have you delivered to the person?
Provides a wide range of services to help individuals maintain their individuality, health, and wellbeing.

Carers and family members play a valuable part in the care of your clients. How have you evaluated and recognised this for the person?

There are a wide variety of healthcare professionals and support workers who work with the adult their families, and advocates to ensure that they get the best possible care. Each member of the Care Team adds a unique set of talents and knowledge to the group.

How did you identify the knowledge and skills of the carer that complement your role as the worker and work in a manner to recognise and support the carer’s relationship with, and knowledge about the person?
The caretaker's critical thinking, communication, and teamwork abilities enhance my job as the worker because they acknowledge and support the caretaker's connection with and knowledge of the individual.

How have you maintained carer and client confidentiality and considered privacy during your interactions with both of them?
Maintaining records and protecting documents may help ensure privacy and secrecy. Most companies have a standard procedure for accessing and storing data and assets. As a result, employees may be required to use access monitoring methods like authorization and pin code access, log books that demand sign-ins and outs. A health professional must adhere to the standards and principles of the Privacy Act 1988 and Freedom of Information Acts in different States/Territories when it comes to protecting an individual's privacy and confidentiality in a health care setting.

What did you do to involve the carer and family when you determined the appropriate support services and delivery options for your client?
A caregiver can offer crucial information about a person's care requirements and demonstrate how to utilise assistive technology or communication tools for those who are unable to articulate their ideas or wants. Assistive workers must make sure that caregivers are fully included in the process of developing a care plan for their patients. Caregivers and those they are caring for must first recognise their connection and establish a strategy around it. Some of the challenges that caregivers may face may be identified, worked with, or overcome. In order to create an effective care plan, support workers may need to engage with caregivers to identify their own strengths as well as the strengths of the person for whom they are providing care.

How did you identify and respond to issues that may impact the physical and emotional health and wellbeing of and, services required by the carer to support the care relationship with the person?
It is important for caregivers to recognise and disclose parts of their work that have a detrimental influence on their personal needs, such as having to constantly get up throughout the night to care for the client, therefore affecting on their sleep. The carer's demands and preferences must be taken into consideration while developing solutions. assisting the caregiver in establishing a healthy way of life

How did you provide the carer and family with information about support services available to them?
Attach evidence as appropriate.
Home and Community Care (HACC) is the primary source of community care for older adults and certain younger adults who want the option of staying in their own homes. Domestic help, food, nursing, transportation, allied health, home maintenance and personal care are just some of the services provided by HACC professionals.

How did you assess the risks that could occur if there was a change to the care relationship and what are some of these risks?
Support workers must be on the lookout for any changes in the care relationship and be prepared to evaluate any new dangers that may surface as a result. Both the caregiver and the individual receiving care and assistance run the danger of bodily and psychological injury if the care connection is disrupted.

What physical and psychological harm could these changes possibly cause the carer and person?
Caregivers who offer hands-on care are more likely to suffer from chronic health conditions such as arthritis, high blood pressure, and long-term back pain. Three reasons caregivers reported contributed most to their stress and worry were not getting enough sleep, giving hands-on care for the individual they care for, and maintaining finances. Depression, tension, and anxiety, stressed family structure, and loneliness and isolation may all result from a lack of social support for those who are vulnerable.

How did you use a strengths-based approach to support the person, carer and family to establish and put in place strategies that utilise positive aspects of change and transition in the best way possible?
