Section 1: Introduction
The overall concept of collaborative care is defined as the process of provision of care services through implementing collaboration with one another while providing the desired level of care or services. Collaborative services are best implemented when the individual or the care receiver is aged and often the communication levels are low in the general cases. In these cases, the collaborative care finds its best implementation (Amare et al. 2020). These are important in aged patients because of a number of reasons.
Firstly, reduced communication gaps are evident as an outcome of collaborative aged care. This allows the patients to convey their areas of comfort and discomfort. Consequently, the care services are positioned or provided in a specific direction. Team Working is another important aspect that is implemented in the case of collaborative aged care. This allows the team working to be improved as well as the levels of services and commitment are also increased.
Secondly, patient-centred care is ensured as an outcome of collaborative aged care. As an outcome of this, the risks evident to the patients are significantly reduced. It increases the efficiency level of the treatment and care process. Better post-operative care is made available to the patient that has been admitted to a healthcare setting, as an outcome of collaborative aged care services. It is evident that the mental health of the aged patients admitted in a healthcare setting is unstable and disturbed.
In this case, the provision of collaborative aged care practices to the patients improves their mental health. This assignment is based on the basic introduction to Mr Walker who is suffering from dementia and is under an aged healthcare program. Different policies enforced by the WHO and Meso policies have been developed that ensures the importance of collaborative care services.
Section 2: Introduction to the Case Scenario
The case study is of an aged person named Mr Rory Walker who has been diagnosed with Dementia, which is a terminal illness. Mr Walker is currently 76 years old and has been suffering from Dementia. Mr Walker has been working previously as a government export officer who was always busy with his work and expressed dedication and true character while working. After retirement, he has worked as a supervising officer in a private construction company for around 5 years.
Mr Walker lived with his wife Mrs Laura Walker and his two children initially. Later after their marriage, they have been living away from Mr and Mrs Walker. Mr Walker had a tormenting past history. He had lost his elder son and elder daughter long ago when they were 16 years and 21 years respectively. He was under psychiatric supervision when he was 43 years old. However, after therapy he was engaged vigorously into work, which had kept his negative thoughts and depression away.
Mr Walker was a sportsman when he was young. He was a baseball player and had once received a concussion in his head, which affected his physical health seriously. However, after prolonged surgery, treatment and care, the condition improved. Later after the retirement of Mr Walker, he had been diagnosed with Diabetes Type 2 and was asked to control carbohydrate intake to control the blood glucose level. He was even advised to use insulin injection to control the diabetic effect.
However, his addiction towards high-glucose food and negligence in medication caused him diabetic retinopathy and diabetic foot. As an outcome of the diabetic foot, he faced difficulty in walking and therefore had to resign from his secondary employment after 5 years. At the age of 65, he was completely locked in his home due to his poor vision due to diabetic retinopathy and diabetic foot.
While being boxed up at his home, Mr Walker hardly went out in a social gathering in his community. He gradually lost touch from his society and neighbourhood and often faced panic attacks. He gradually developed mental illness, as he became maniac...
