Page 3 of 8 [854] Assignment Task Case Study You are a nurse at Guy’s and St Thomas NHS Foundation Trust working with patients with lung cancer. A patient in your care has a medical condition of cancer of the lungs with a very poor prognosis where the health and care professional team think she is at the end stage and does not have very long to live. Mrs. Wang has had symptoms of breathlessness, loss of weight and a persistent cough. Mrs. Wang’s first language is Mandarin and although she does understand and speak some English, she is rather hesitant to speak it and prefers to discuss her health and well-being in Mandarin. Mrs. Wang’s two sons have accompanied her to her health appointments throughout her treatment and have provided some interpretation for her to communicate in Mandarin. Mrs. Wang’s two sons and the rest of her extended family have requested that Mrs. Wang not be told about the nature of her diagnosis at all but rather tell her there is little wrong and that the tumour is non-malignant. Q1. This case study is a situation of ethical dilemma where the patient, Mrs. Wang is not made aware of her actual medical condition. Mrs. Wang's first language is Mandarin and is not very familiar with the usage of English. Mrs. Wang’s family members and acquaintances have suggested to not let her know about the actual medical condition. As the nurse at Guy’s and St Thomas NHS Foundation Trust, not making a patient aware of his/her actual medical condition violates the ethics. Ethics by definition is a philosophy that involves defending, systematizing and recommending the concept of wrong and right conduct. The nurses or staff are expected to perform their duties ethically. In addition to their work responsibilities, they are expected to recognize the situation of ethical dilemmas and take the right decision for every situation. They should also inform staff members or other members of the organization about any issue which will affect the client. There is also a code of ethics which are standardized norms every nurse has to abide by to keep on continuing their practices(Fry, S.T., Johnstone, M.J. and Fletcher, M., 2003). They are also expected to promote ethical practices by evaluating the outcomes of interventions. The ethical principles that the nurses have to adhere to our principles of beneficence, justice, nonmaleficence, fidelity, accountability, veracity and autonomy. The principle of beneficence states that right and patient-benefitting actions should be taken for a patient. The principle of justice in fairness states that nurses should be indifferent to patients and distribute care which is just, fair and equitable to all. The principle of nonmaleficence states that no intentional or unintentional harm should be done to the patient as per the Hippocratic oath. The principle of accountability states that nurses are accountable for their actions. They must accept the personal and professional consequences of their actions. Then comes the principle of fidelity, in which one has to keep the promise they make(Duff, R.S. and Campbell, A.G., 1973). The nurses should try fulfilling all the professional promises they have made to the patient such as providing safe care and high-quality service competently. Autonomy should be maintained and the patients should be encouraged to make their own decisions. The nurses should always suggest the right thing that is to be done, but the patient should be free and independent to take the decision. The patient has the right to reject or stop the treatment anytime they want. The nurses should maintain veracity, where they should be completely truthful with the patient and let them know their true condition and not withhold the truth that can lead to distress from them. In this case study, the client is not made aware of the actual health condition. Though the family knows the actual situation, the things are hidden from the patient. The situation of Mrs. Wang is critical and does not have much time to live. Ideally, in this situation, the patient should be made aware of the conditions so that they are mentally prepared and can plan out all the important work they have to do. There is also a communication gap because of language constraints. Since the family members do not want Mrs. Wang to know the truth I believe the nurses should not tell her the whole situation. They should respect the family’s motive as well. We should make the family understand that it is the patient’s right to know about his/her medical condition. We should make them aware of the consequences of the ethical procedure is not followed. Mrs. Wang is suffering from lung cancer, so for such patients, the treatment itself needs a lot of physical and mental strength and motivation to keep going on. Once Mrs. Wang will know the condition she will understand the depth of the situation and will be able to make a better and informed decision about her own-self. Every individual’s choice must be respected. Though the family may not see the need for her to know things it may be a very important thing for Mrs. Wang. The family may fear that Mrs. Wang would stop taking the treatment or get more demotivated or make choices that are not in her benefit then we may look at negotiating proper solutions. To treat a person with dignity we must listen to their concerns. They should feel that they are valued. We should often ask them about their views or opinion to show them how important they are and will always be. They should be involved in as many decision-making processes as they can be(Kälvemark, S., Höglund, A.T., Hansson, M.G., Westerholm, P. and Arnetz, B., 2004). They should be included in conversations and their suggestions or opinions should be considered. Mrs. Wang is an old lady who knows what is right for her and what is not. She is capable of making her decisions and she should be respected for the choices she makes. She should be made aware of the reason why she is receiving care and how can things be cured. This makes the person feel connected and valued for the whole process. The lack of control over the situation is dehumanizing. She may feel that others are having complete authority over her life. They should be involved in all the discussions that have a direct or indirect linkage with their care. The strategy should be advising her family members to tell Mrs. Wang the truth. While for Mrs. Wang it should be making her feel valued and the communication is done in Mandarin which will make her feel more comfortable. The usage of her home language may seem a small or trivial thing to do...
