This assessment is to be undertaken in a fully equipped aged care facility.
For this assessment, you are to be observed on 3 separate occasions (2 observations by your workplace supervisor and 1 observation by your assessor). For this activity you are required to demonstrate your ability to provide support to 2 different people living with dementia.
You must ensure that you provide evidence of your ability to provide support to people with dementia including:

Using person-centred care approaches with clients

Understanding and referring to the client’s care plan when necessary

Creating and maintaining a suitable environment for clients with dementia

Report to supervisor as necessary

Use different communication strategies in different situations

1. Mavis suffers from severe dementia
Your client Mavis suffers from severe dementia. Prior to her diagnosis, she participated in many social activities including the country women’s group, line dancing, and regularly attending church. But after the diagnosis, her condition worsened to the point that she could no longer live in her own home and she now lives in an assisted living facility. She regularly forgets what day it is and gets frustrated over her unfamiliarity with time.
a. In what ways could you assist to make the transition from home to the assisted living facility easier for Mavis?
It is not easy for Mavis to transit from her normal routine to that of a dementia patient. She is suffering from severe memory loss that is surely frustrating for her. It is my responsibility to make her comfortable at the assisted living facility by firstly giving her a short visit. She must be allowed to sit for a while at her favourite place of the facility regularly to make her happy. I will inform others to talk to her calmly and talk to her. She surely needs emotional, and physical assistance. She needs someone to regularly take care of her needs. I can offer her dance, and socialize with other residents to make her more comfortable at the assisted living facility.
b. How can you find out more information about Mavis?
I can find out more information about Mavis from her individualised care plan. This care plan assessment provides the details of the patient to the health care providers. It provides more assistance various information including the medical diagnosis, allergies, behaviour, bladder, and bowel movement, communication and sensory, hygiene, medication, mobility and transfer, nutrition and hydration, oral health, pain, interests, technical care, and toileting. These diagnoses along with the interventions are helpful to the health care providers to provide specific help to dementia patients. I can talk to her family members, neighbours, and colleagues to get an insight into her earlier routine and interests. It is necessary to have collective information on the physical and social well-being of the patient.
c. Give two examples of how you would ensure person-centred practice in this scenario.
There are many principles associated with the person-centred practice of people suffering from dementia.
· Firstly, it is necessary to know how the personality and lie experience the people affect them while living with dementia. For example, Mavis participated in many social activities like line dancing, and country women’s groups. This made her happy and allow her to live amongst the community member. To deal with this, I can ensure some social or community groups that she can attend. It will help her have a good daily routine and be relaxed.
· Secondly, it is important to know the perspective of Mavis. I need to talk to Mavis about her life at the assisted care facility. If she likes sitting in the sun during the day or doing exercise or going for a walk, I can do my best to give her those facilities. I can let her visit a park with an assistant, or read something on a chair on the lawn.
d. When communicating with Mavis, what are some communication strategies you can use in order to gain her trust
It is important to use some communication strategies to have better communication with Mavis:
· I will speak slowly and clearly to her by using short sentences.
· I will always talk by making direct eye contact to gain her trust.
· I will give her enough time to give aresponse, without any pressure.
· I will ask her to talk to other residents and get involved in the conversation.
· I will listen to her completely and encourage her to speak more.
· I will give her choices to opt from, but it will not include complex choices.
· I will keep my tone positive and friendly.
· I will keep a respectful distance from her while talking.
· If she is comfortable, I will hold her hands to assure confidence in her.
e. Describe what activities you could arrange for Mavis in order to enhance her self-esteem as well as allow her to maintain as much independence as possible.
It is necessary to arrange some activities for Mavis to promote her self-esteem, compensate for lost abilities, and have opportunities to enjoy, and socialize with others. Some of those activities and exercises are:
· Dancing
· Listening to music
· Stroking animals or differently textured materials
· Brushing her hair
· Visiting a herb farm
· Gym work and aerobics
· Cycling
· Gardening
· Vacuuming
· Folding laundry
f. In what ways could you address any possible stigma in relation to this scenario?
It is necessary to address the stigma associated with dementia, and how to deal with the patients. This stigma can be dealt with by:
· Awareness
· Support after diagnosis
· No assumptions
· Positive language and help to the patients
· Helping the patients to socialise
· Let them join a support group
· Avoid self-doubt and shame
· Let them perform small activities to give a sense of independence
2. Beth goes for a check up
Beth is your 67-year-old client who you are taking to the GP for a medical check-up. You arrive at the house to find her in the backyard arguing and hitting her husband with her hand and then with a newspaper. She is accusing him of having a lady friend staying in the house. She appears confused and aimlessly wandering in the garden, incessantly moving pot plants around and accusing the next door neighbour of stealing them.
a. List the behaviours of concern exhibited by Beth.
Behaviours of concern exhibited by Beth are:
· Mistrust
· Violence – physical and verbal
· Confusion
· Aimlessness
· Lack of concentration
· Lack of coordination
· Baseless accusation
b. Which behaviours if exhibited by Beth would you consider a risk to yours and other’s personal safety?
The behaviours that are risky to my and others’ safety are violence. Under such conditions, Beth might harm anyone physically even if she didn’t mean to, especially her husband with whom she lives and doesn’t trust. This will keep others’ under their toes and she might not be able to get community help. Another behaviour of concern is false accusations because, after this, her neighbours might not even talk to her and provide her help if she needs it.
c. Using your workplace procedures as a guide, discuss the management strategies you have used to manage behavior of concern with a client with dementia. Include in your answer:
· Its effectiveness
· Any assistance provided before during or after
· Evaluation of the management strategy
· Future plans
· Refer to the needs driven behavior model
Concerning behaviours of the dementia patients according to the driven behaviour model are:
· Wandering
· Sundowning or restlessness
· Agitation
· Anxiety

Some management strategies to deal with dementia patients according to the guide provided at my workplace procedure:
· Make a schedule for exercise
· Keep the objects away that may result in wandering
· Decide a safe place for the patient to move freely
· Arrange a chair or bed alarm
· Ask the patient to do activities to calm down them
· Avoid showers in the late afternoon
· Identify the cause of anxiety and try to resolve it
· Reassure the patient by holding hands or showing empathy
· Use a low tone and remain calm while talking to the patient
· Never force the patient for any task or activity
· Don’t argue with the patient or get offensive of their accusations
· Don’t trigger the cause of anger or hallucination
· Maintain a familiar atmosphere with a proper routine
· Provide them prescribed antipsychotic
· Remind the patient of inappropriate behaviour
· Give them comfortable clothes to wear
· Give response gently and never overreact
· If the situation is unavoidable, ignore them

There are many potential triggers for the unusual triggers of dementia patients:
· Psychological triggers – Dementia patients are unable to express their emotions or pain verbally. They express unease with their behaviour.
· Medical triggers – This includes side effects of medicines, fever, bruises, ache (toothache, headache), sores, or wounds.
· Environmental triggers – It includes separation from loved ones, isolation, sensory overload, uncomfortable clothes, extreme temperature, bright light, mirrors, shiny floors, and colour contrast.
