Critically discuss the roles and responsibilities of other care professionals and organisations within complex case management in the co-ordination of care. Do this by critically analysing, appraising and evaluating the dynamics of the chosen management intervention and the involvement of others. Ensure this includes a critical review of the role of the nurse.



Include relevant theories and strategies relating to areas such as communication, team dynamics, partnership working, inter-disciplinary team working and integrated care delivery. Critically review the extent to which the service user and family/carer perspective was integrated into the coordination of care.


From a holistic perspective, critically evaluate the co-ordinated care. Consider this in relation to the client group and service user, was the intervention appropriate:
