These conditions have been identified as National Health Priority Areas according to the Australian Institute for Health and Welfare. This assignment will also help you demonstrate effective communication in oral and written English language and visual media. Learning outcomes assessed: LO2, LO3 How to submit: Submit your Vodcast online through LEO by 11:59pm Sunday 25th September 2022 to the appropriate submission box under the ‘Assessments’ folder. Return of assignment: Marks to be released via the BIOL122 LEO site within 3 weeks of the assessment due date Assessment criteria: Marking rubric (at the end of this document) 2 Detailed description:  This assessment task requires you to make a 7-minute Vodcast presentation (a PowerPoint presentation with audio, written text, and pictures/diagrams) about diabetes mellitus (Type I, Type II or gestational diabetes) or asthma. The Vodcast will be assessed according to the following criteria: o 1. Depth subject matter, o 2. Accuracy of the information, o 3. Clarity of presentation, o 4. Ability to engage the audience, o 5. Overall quality, o 6. Adherence to time, o 7. Referencing  Your Vodcast has to be about a specific ‘person/client’ who has diabetes mellitus (Type I, Type II or gestational diabetes) or asthma.  You will need to create your own ‘person/client’ according to the following criteria*. ‘Person/client’ criteria For example For example Fill in for your ‘person/client’ What is their name? Phillip Nguyen James Mae What is their gender? Male Male What is their age? 58 years old 18 years old What condition do they have? (diabetes or asthma) Type II diabetes mellitus Asthma Are they within a healthy weight range? Overweight Underweight Do they exercise? Not any more; used to take short walks every day Rides bike a couple times a week to University What is their occupation? Retired accountant Student What is their living status? Phillip lives with his wife James lives alone What is their family medical history? Father died of complications related to diabetes mellitus Mother suffers from seasonal asthma Are they pregnant? N/A N/A Are they a smoker? Smokes a packet a day No What is their alcohol intake? Enjoys 2-3 glasses of wine with dinner each night N/A What medications or treatments have they been prescribed?# Metformin Sulbutamol *The examples listed in the table above are suggestions; you need to create your own ‘person/client’. 3  You will then be required to answer a series of specific questions (listed at the end of this document) that relate directly to your ‘person/client’ and the condition they have. All of your answers must be specific for this person; marks will not be given where your presentation does not relate to the person you have ‘created’.  The specific questions are listed below.  Vodcasts are expected to stay within the 7 minute time limit. Presentations exceeding 7 minutes will incur penalties as per the marking rubric.  When submitting your Vodcast presentation to LEO, make sure you adopt the following naming principles: SURNAME_FIRST NAME_TOPIC. For example, if John Doe created a presentation on Asthma, the file name would be “Doe_John_Asthma”. For further details, see checklist below.  You will need to research your topic; please ensure that you keep a record of any references that you have used.  You will be required to write an annotated bibliography in apacontaining 5–10 citations as part of this assignment. An annotated bibliography provides information regarding how you used the research to support your assignment.  APA7 Referencing style is to be used for this assessment For example, you may have used the following journal article: Wu, Y. and Ding, Y. (2014). Risk factors contributing to Diabetes. International Journal of Medical Sciences 11:1185-1200. I used this article to examine the risk factors for diabetes mellitus. It is a reliable source as it is peer reviewed. To create your assignment:  Create your Vodcast with PowerPoint using the audio function (do not turn video on). The following video from Lynda.com explains how to embed audio into a PowerPoint file: http://www.lynda.com/Office-tutorials/Embedding-sounds/85084/90680-4.html  It is expected that you will use some figures or visual aids to help enhance your presentation.  Your Vodcast should not exceed 70 MB. You will not be able to upload your Vodcast file to LEO if it is larger than 70 MB.  The Vodcast needs to be your own original work; you cannot include YouTube clips or other videos as part of your Vodcast, but you can generate your own video files.  Please be mindful that it can sometimes take 10–20 minutes to upload and submit your Vodcast file to LEO; please do not leave submission until the last minute.  It is your responsibility to check that your Vodcast has been submitted successfully. To do this, you will need to go to a different computer, log onto LEO, open your Vodcast, and check that you can see the visual material and hear the audio content. We have had submissions without (working) audio in the past, which meant that these submissions attracted only a fraction of the total marks. 4 Check-list for this assignment ☐ I have chosen the right condition (Type I Diabetes Mellitus, Type II Diabetes Mellitus, Gestational Diabetes or Extrinsic Asthma). ☐ I have created a ‘person/client’ based on the criteria listed in the table above. ☐ I have researched the condition that my ‘person/client’ has been diagnosed with. ☐ I have answered the specific questions that were made available on LEO, and all my answers are related to my particular person/client. ☐ I have used the answers to these specific questions in creating my Vodcast. ☐ I have used an annotated bibliography at the end of my presentation. ☐ My Vodcast does not exceed 70 MB. ☐ My Vodcast does not contain any YouTube clips or videos other than those I have personally created. ☐ My Vodcast is within the required 7-minute duration. ☐ I have named my file in a way that indicates the Vodcast topic (e.g., Doe_Jane_Asthma) ☐ I have checked my submitted Vodcast file using a different computer; the visual material is working, and I can hear the audio content. ☐ I have read the marking scheme outlined in the Vodcast information on LEO (also listed at the end of this document). 5 STUDENT INSTRUCTIONS: VODCAST ASSESSMENT You will have the opportunity to describe the pathophysiology and pharmacological management for a person/client with type 1 diabetes mellitus, type 2 diabetes mellitus, gestational diabetes or extrinsic asthma. We expect you to show your understanding by making explicit the links between the pathophysiology, clinical manifestations and pharmacological management of the person/client’s disease; your target audience is health professionals (e.g., Nurse, paramedic, midwife. At the beginning of your Vodcast, you need to set the scene within the first minute by introducing your person/client. Following this introduction, proceed to answer the following questions. DIABETES MELLITUS Question 1a Explain how BOTH of the following are different in your patient in comparison to a healthy, non-diabetic person. (i) insulin synthesis and secretion and (ii) insulin binding to its target cells Question 1b Describe the aetiology and pathogenesis of the type of diabetes your client has and compare them to either of the other two types of diabetes. Question 2a Both metformin and insulin may be used in the management of diabetes mellitus. By making meaningful references to your patient, (i) Choose the one that is the most appropriate for the treatment of your patient, (ii) Describe its mechanism of action, and (iii) Explain why the drug you selected is the most appropriate for the treatment of your patient. Question 2b Explain the significance of HbA1c measurement in your client and what a high reading would indicate. Question 3 Explain the link between long-term poorly controlled hyperglycemia and ONE of the following: (a) nephropathy or (b) hypertension. 6 EXTRINSIC ASTHMA Question 1a Two of the major pathological changes asthmatics face are inflammation of and increased mucus production in the airways. By reflecting on the pathophysiology of bronchial asthma, explain how these pathological changes arise in your patient. In your explanation, you are expected to include a brief discussion of the immune cell-mediated response. Question 1b The clinical manifestations of an acute asthma attack may include the following signs and symptoms: GROUP A GROUP B Cough Airway oedema Hypoxia Increased respiratory rate Wheeze Increased heart rate Tight chest Dyspnoea Choose one clinical manifestation from GROUP A and one from GROUP B, and then relate them to the underlying pathophysiology of your person. In your answer, explain the link between an acute asthma attack and both signs/symptoms you chose. Question 2a Relievers and preventers are used in the management of asthma. Provide an example of each, explain the mechanism of action of these two drug classes, and describe their benefits in managing your patient’s condition. Question 2b Adequate education of patients suffering from/prone to asthma attacks is important in asthma management. Briefly summarize the advice you would give to your patient in respect to i. Corticosteroid administration and ii. Strategies to effectively prevent and manage their acute asthmatic episodes. Question 3 Explain the pathomechanism of the following long-term consequences of extrinsic asthma and their impact on your client’s quality of life:  Bronchial smooth muscle hypertrophy  Subepithelial collagen deposition 7 BI O L1 22 (H um an B io lo gi ca l S ci en ce 2 )— Vo dc as t m ar ki ng r ub ri c St ud en t n am e/ ID : M ar ke r: 85 –1 00 % 75 –8 4% 65 –7 4% 50 –6 4% 0– 49 % M ar k D E P T H ** Q 1 Th e an sw er is co m pl et e, an d al l as pe ct s o f t he p ro bl em a re co ve re d in e no ug h de ta il Th e an sw er h as o nl y a fe w m in or o m iss io ns Th e m ai n as pe ct s o f t he qu es tio n ar e co ve re d, b ut th e an sw er sh ou ld h av e be en m or e de ta ile d Th er e is on e m aj or p oi nt m iss in g fr om th e an sw er Th e qu es tio n is no t a dd re ss ed at a ll, o r t he re a re se ve ra l m aj or p oi nt s m iss in g /1 0 Q 2 Th e an sw er is co m pl et e, an d al l as pe ct s o f t he p ro bl em a re co ve re d in e no ug h de ta il Th e an sw er h as o nl y a fe w m in or o m iss io ns Th e m ai n as pe ct s o f t he qu es tio n ar e co ve re d, b ut th e an sw er sh ou ld

Diabetes Mellitus Type 2
Introduction of Client
My client is a 67-year-old Asian female, had diabetes mellitus Type 2 (T2DM) for the last 30 years. She is overweight (102 kg) and no longer invo
lved in any kind of physical exercise. Earlier she used to walk a few steps, now she cannot walk due to pain in her knees, and legs (Neuropathy). She walks with the aid of a stick while doing her household chores and lives with her husband. Both parents had diabetes mellitus type 2. She does not smoke and does not take alcohol. She has been taking Metformin medicine for the last 30 years to control her blood sugar.
Insulin is produced in the pancreas and acts with glucose to regulate energy in cells. The client has diabetes mellitus type 2, which is a chronic metabolic condition that develops through consistent hyperglycemia.
1A(i) Insulin Synthesis and Secretion
Insulin secretion and insulin resistance are impaired in people with type 2 diabetes mellitus. Chronic hyperglycemia and metabolic aberrations cause deterioration of multiple organ systems. A few complications include retinopathy, nephropathy, neuropathy, and microvascular manifestations. Pears is having pain in her legs and a piercing sensation in her feet. She has a problem of blurred vision. She had undergone surgery for that two years ago. She also had a recent fall and her right shoulder was fractured. It took 1 month to repair after plaster. In Diabetes, bone healing and wound healing take longer time than in healthy people. In Type 2 diabetes mellitus, people are at risk of cardiovascular illness.
In healthy people, beta cell stimulation and release of insulin support the uptake of glucose, fatty acids, and amino acids through insulin-sensitive tissues.
In T2DM, the body does not synthesize sufficient insulin. Insulin is required to process glucose in the food to produce energy. Abnormal increase in blood glucose levels disrupts glycemic control. Beta cells show...
