Assessment description

Students will also analyse communication in health care and reflect on their own capabilities for health communication.

Assessment instructions

You are preparing to care for Pauline during a community nursing visit:

  1. Review Pauline’s case below andconsider the patient/person

Pauline-1.PNG

https://www.freep.com/story/life/advice/2016/03/08/ask-amy-lonely-middle-aged-woman-wants-friends/80961282/

Pauline is a 48 year old woman who lives alone in her 2BR government housing unit in Canberra. Pauline has a history of diabetes and anxiety

,

which are managed by

prescribed medication

s

, she has no surgical history. Pauline trained as a teacher’s assistant, however, has not been employed for 20 years due to her social anxiety.

Pauline is currently receiving Community Nursing visits at her home twice per week for wound assessment and dressing after sustaining an injury to her right shin. The wound resulted from a deep scratch from a rosebush thorn while gardening in her small courtyard 6 weeks ago; the wound initially did not stop oozing even with a basic dressing (bandaids) applied. After her injury, Pauline waited until she could get an appointment with a bulk billing (due to financial pressures) General Practitioner close to her home, this was 3 days after the injury.

Pauline has also recently suffered bereavement and has not been motivated to engage in her normal activities/hobbies and has stayed primarily indoors necessitating in home care.

This week, as a student nurse, you are attending Pauline’s home with the Community Nurse. When you meet Pauline, she is sitting on her couch, she is not maintaining eye contact and is replying quietly ‘yes’ or ‘no’ responses to the Community Nurse’s questions about her wound. Pauline’s clothing is loose and appears to be stained. You observed a pile of dirty clothes on the floor in the laundry as you walked in.  The nurse goes to the kitchen to prepare the equipment for the wound dressing and asks Pauline is she would like a cup of tea, Pauline replies “No, I have no appetite and no milk in the fridge”. The Community nurse invites you to have a conversation with Pauline regarding her wellbeing.

2. Research health assessment frameworks and health interviewing related to psychological, social and emotional health; prepare a set of questions for a health interview focused on psychosocial/emotional health needs.

3. Engage in a conversation (we suggest around 10mins) with AI patient Pauline

using theSimConverse platformLinks to an external site.

(an exemplar video will be provided of this, and a practice AI patient will be available for students to interact with). Please be reassured that the content of your AI conversation is not marked, it doesn't matter if you forget to ask certain questions, or miss bits of information - it is for your formative learning, practice and reflection on health interview and communication. After your conversation with AI patient Pauline, download a copy of your conversation transcript to upload with yourassessment.HereLinks to an external site.

is an instructional video on how to download your transcript as a PDF.

The Clinical Reasoning Cycle (Levett-Jones, 2018) is the underlying framework used for the development of this assessment. You will be marked on the following components of your assessment - please develop these in the

template provided.

Download template provided.

  • From the existing case data and your AI conversation data, identify at least five (5) patient

    cues

    related to psychosocial/emotional wellbeing (

    ULO1)

  • Using the health data gathered, develop three (3)

    actual and/or potential health issues

    statements

providing a rationale for each statement supported by evidence (

    ULO2)

  • Using relevant health/nursing literature describe two (2) evidence based therapeutic (safe and person-centred)

    nursing goals of care

based on the actual/potential health issues identified from the assessment data analysed (

    ULO 2,3)

  • After your community nursing shift, you are preparing to discuss your clinical assessment findings with a nursing supervisor. Read and consider the materials provided on Canvas related to intraprofessional communication (seeAssessment 2 resourceson Canvas). After reading and considering these materials address the prompts in the ‘reflection section’ of the template. Your responses (analysis) to the prompt questions need to be supported by evidence indicated by intext citations (maximum of 750 words including intext citations). These evidence informed responses will include reflective content indicating learning for nursing practice (

    ULO 5)

to >1.25

Pts

Satisfactory

At least five (5) health cues are identified that are somewhat relevant to the case information provided or elicited during the health interview with the patient; the cues idenitifed demonstrate satisfactory understanding of the wellbeing of the patient; there may be some repetition/lack of specificity in the cues identified

1.25

to >0

Pts

Below Expectations

Insufficient relevant cues identified; AND/OR cues identified are general rather than specific to the case provided and/or include significant repetition; AND/OR cues identified are not related to the wellbeing of the patient

Introduction
Patient Cues Related to Psychosocial or Emotional Well-being
1. Pauline's lack of eye contact and minimal verbal responses.
2. Loose and stained clothing, suggesting neglect in personal care.
3. Pile of dirty laundry indicating difficulty with maintaining daily tasks.
4. Lack of appetite and no milk in the fridge, possibly reflecting financial struggles or decreased motivation (Melnyk & Fineout-Overholt,2022).
5. Withdrawal from regular activities and hobbies post bereavement.
Health Issues Statements
1. Social Withdrawal and Isolation - Pauline’s recent bereavement, lack of motivation, and staying primarily indoors suggest she may be experiencing social withdrawal. This can result in reduced mental and physical well-being. (Melnyk et al,2021)
2. Neglect of Personal Care - The observation of stained clothing and the pile of dirty laundry hints at Pauline neglecting her personal care, which can be a symptom of depression or severe anxiety. (Park et al,2020)
3. Decreased Appetite and Possible Nutritional Deficiency - Pauline’s statement regarding her appetite and lack of basic food items might indicate nutritional deficiencies that could exacerbate her physical and mental health conditions (Clarke et al,2021)
Evidence-Based Therapeutic Nursing Goals of Care
Therapeutic Nursing Goals of Care
1. Promote Mental Health and Emotional Well-being
- Description: With Pauline's signs indicating possible depression following bereavement and her exhibited social isolation, it's essential to address her emotional and psychological needs. A holistic approach that combines therapeutic communication, counselling, and possibly medication can benefit her.
- Intervention Strategies:
