Task description: The assessment tasks requires you to write an academic essay that addresses the following tasks: 1. From the chosen case study identify and discuss two (2) signs or symptoms of clinical deterioration reported by the family member, making connections between the related pathophysiology and the patient’s reported admission diagnosis. (400 words) 2. Identify one (1) priority problem associated with the patient’s clinical presentation and signs or symptoms of clinical deterioration (identified in point 1). Justify why the selected problem is a clinical priority within the case. Use contemporary research to support your justification. (500 words) 3. Discuss two (2) evidence based clinical interventions to address the priority problem and how to evaluate the efficacy of these interventions. (500 words) References: • The reference list is to be presented in accordance to QUT APA 7 th edition requirements. • References are to be contemporary (i.e. no older than 7 years). • Only peer reviewed evidence-based literature are to be cited for this assessment. Additional information: • There MUST be two (2) clinical interventions discussed to address the priority problem. • Of the two interventions, a MAXIMUM of one (1) intervention can be solely related to a pharmacological agent. The second intervention may be nurse initiated and led and/or a collaborative intervention. • All interventions should identify and consider the nursing role within the provision of the intervention. Please note: • If using web pages they must be current and evidence based. Sponsored sites and review sites such as StatPearls should be avoided. Your textbooks can be used but should not make up the majority of supporting evidence. • The submitted essay should NOT contain tables, figures or appendices. • The use of dot points is NOT permitted. • Heading and/or subheadings are NOT permitted in this essay. From the chosen case study attached in the file, two major signs or symptoms of clinical deterioration identified by the family members are firstly poor breathing problems, coughing problems that leads to asthma at
tack and tracheostomy and secondly, non-focal seizures related problems that is caused by hypoxia. These are two major health related issues associated with this particular patient Adam in the case study.
Adam is just of 17 years old boy who is being admitted to the respiratory ward from ICU with a diagnosis of hypoxic brain injury. Adam already had been inserted tracheostomy whilst in ICU to assist with the clearance of the secretion due to poor cough strength as well as increased secretions and thereby has been kept in mechanical ventilation.
The muscles around the troubled airways contract and hence the airways produce extra mucus causing the breathing tubes to narrow. During this attack, patient might feel like coughing more often, wheezing, sneezing and faces breathing problem. There are several types of asthma, but Adam posses’ acute asthma attack, as this occurs after the hypoxic brain injury (Kostakou et al., 2019, 1283).
Due to this, he suffered from poor cough strength and increased secretion and for this he has been kept in mechanical ventilation, where patients can breathe properly with the support of machines. The patient here is connected to the ventilator with a hollow tube that goes into their mouth and down into the main airways. Due to all these affects patient Adam is coughing more and is feeling cool as reported by his parents and family (Bush, 2019, 68).
Secondly, Adam was suffering from non-focal seizures, which is caused by hypoxia and presently is controlled with sodium valproate. Non-focal seizures arises where the main symptoms do not involve muscles activity. This includes the emotional changes, changes in thinking capacity as well as in sensation. Through neuroplasticity, brain injury survivors can make astounding recoveries. Nevertheless, as argued by Galetto and Sacco (2017, 803), still, the dead brain cells cannot be brought back or reverse a brain injury. Huge and sufficient improved oxygen is needed in this regard.
From the case study it is being noted that Adam was been admitted to the respiratory ward six days after three weeks of admission in ICU, due to asthma attack. During the ICU period he was been diagnosed with non-focal seizures caused by hypoxic and is being controlled with sodium valproates. Further, he has been shifted to mechanical ventilation section due to poor oxygen level and terrific breathing problem (Zhou et al., 2017, 1650).

Clinical priority setting means, it is a process where the physicians or the nurses undergoes a selection of what to investigate, what and when to proceed with the diagnostic tests, when to start the treatment, when to give medicines and many more (Suhonen et al, 2018, 28). There are some diagnostics or medical tests, which are being done before and some are done afterwards.
Priority tasks are those, which are to be sorted beforehand in order to continue with the medical procedures or treatments. Therefore, before undergoing the treatment procedures, priorities should be analysed. There are certain situations where priority problems arise as the healthcare departments or the physicians or even the nurses fail to undergo proper decision, with which to medical treatments or test proceed and which not to.
During these selection or decision dilemma phase the patient...
