Patient, Family, or Population Health Problem
I'm helping Mr. DM, who has hypertension, to better control his condition. Maintaining a healthy lifestyle might improve his overall well-being by lowering his blood pressure, helping him lose weight, making it easier to move about, and lowering his blood sugar levels. To control his hypertension, Mr. DM will exercise for 25 minutes every day for one week and eat a lot of fruits, low-fat dairy, nuts, and legumes. He will also cut back on grains and increase his consumption of veggies. His sodium intake, red meat consumption, canned veggies and processed meat consumption would be restricted. Mr. DM will be given time to adjust to the adjustments before they are implemented in full. In order to save money, Mr. DM intends to cut back on his medicine and appointments with the doctor. As a result of a healthier lifestyle, the likelihood of being hospitalised is reduced. Mr. DM's condition is to be improved so that he can resume his normal activities. I decided to create an educational handout for Mr. DM and his family in order to help them better understand and manage the patient's condition. Since Mr. DM will be able to monitor his blood pressure using the handout, he will benefit from it. Handout images will help remind him of the necessary safety precautions. Mr. DM's family members will also keep an eye on his progress and alert the caretakers to any issues they see (Almost et al., 2020).
Role of leadership and change management in addressing the issue
Nurses should inform patients and their families about diagnosis, nutrition, exercise, and medicine, keep track of the patient's condition, and handle any issues that arise so that Mr. DM doesn't have to deal with any further problems as a result of his treatment. A nurse leader has a responsibility to be a role model in the nursing department in order to improve patient outcomes by coordinating all of the key duties of the nurse. Patients, their families, physicians, and other members of the health care team all benefit from nurses' efforts to enhance communication. For patients and other multidisciplinary teams, nurse leaders would also play an important role (G.Morgan et al., 2005).
