Clinical Scenario You are working in a community clinic as a registered nurse. Your next patient is Ms Florence ‘Flo’ Ljukuta and her husband Lionel. Flo is 70 years old, and Lionel is 84 years old. Flo is the primary carer for her husband who has physical disabilities due to a stroke and was recently diagnosed with dementia. Flo would normally attend the clinic regularly, but you notice in her records she has not presented in nearly 12 months. The last entry on the eHealth record is a hospital visit due to a fall last year. When Flo and Lionel arrive, you notice that Flo is limping and they both appear underweight. You also notice Flo and Lionel looking untidy wearing clothes which appear to be dirty, which is very unusual for them. You read both Flo and Lionel’s medical file and identify no services are currently linked to them and Flo is looking after a lot of family in her house. When you ask Flo, what has brought her in, Flo tells you she has a wound on her right foot which she has been dressing at home for three months, but it is not getting better. Flo tells you she hasn't seen a doctor about her foot earlier because it was only a small wound and she didn’t want to trouble anyone. Flo also stated because she doesn’t drive, she is finding it difficult to catch public transport with Lionel. She can’t leave him at home because he sometimes becomes agitated and upset plus the children are so busy looking after their little ones, she doesn’t want to bother them. Aged care services and interdisciplinary care to maintain independence
Aged care services/packages
Aged care services offer multidisciplinary hospitality and community care services. They provide support through coordination between the community and the hospital. They provide services to aged patients with a quality life supporting them to live with utmost independence. These serv
ices provide end-of-life care to older people (Chan & Chan, 2018). The two aged care services/packages identified are discussed below:
1. Home aged care service
Home care service relies on providing quality care at home based on the patient’s condition and requirements. Home care services deliver independent assistance and care with the best quality treatment. They have become important in recent days due to the increase in the elderly population and their cost-effective nature (Schnelli et al., 2021). It mostly provides extended independence to aged people by providing them comfortable, safe, and social life at their homeplace. They offer one-on-one, personalized care to the patient granting their family members varied opportunities to reduce their stress levels. This program provides a tailored and well-structured service package enabling people to remain at home. Number of people using this service has found to increase enormously in comparison to other aged care services.
The most challenging fact with the diabetic patients is to maintain a healthy lifestyle and to manage blood insulin levels. This can be overcome with proper medications, diet, and physical activity without which the disease can be more dangerous. Thus, home care services provide a solution to reduce its effect on the health of the diabetic patients. Thus, they will be offered an expert set meal plan, proper medications, personal hygiene, and exercises. Diabetic patients and their families also receive education and assistance from their caregivers. The patients with diabetic wounds and diabetic-related complications are most benefited from their services.
Referring to Flo’s case, as she is already suffering from diabetes. She also presents a diabetic foot wound and seems underweight. She requires proper diabetic management, proper diet, and proper care which is possible through home care services. Also, as observed Flo finds it difficult to maintain proper hygiene. Thus, the caregivers may help Flo in managing diabetes, have proper food, take care of the foot, and engage in physical activities. Flo can also get washing and cleaning services for cleaning and laundry facilities as well as food preparation services. Although Flo’s land is in the remote area she resides in a town. Thus, she can have complete access to home care services through their contact by online search.
2. Residential aged care service
Residential services provide support for people who require assistance with day-to-day tasks and healthcare. These services usually help needy people. Moreover, 2,44,363 Australians of older age lived in residential aged care facilities between 2019-20 (Khadka et al., 2021). They provide accommodation, meals, and food, personal care, basic healthcare services as well as palliative care. Apart from these services, they also provide allied therapies like podiatry, occupational therapy, and physiotherapy to maintain mobility in patients. Nursing care services are provided to manage medical conditions. Also, specialized support services are offered for people with vision problems, dementia, etc. They provide beneficial services in a home setting thus maintaining the independence of the elderly patient. Thus, they offer peace of mind to the patient’s family and make them more satisfied.
As Flo presents medical conditions such as diabetes, hypertension, asthma, angina, hypercholesteremia, osteoarthritis, etc. residential aged care services can provide a perfect solution for the condition. As Flo is underweight, a nutritionist may help manage her weight. Also, residential eye care services have decreased the prevalence of blindness in older people (Khadka et al., 2021). Apart from this, an occupational therapist may help Flo in following a routine physical activity. As Flo has foot-related problems, a podiatrist may help her to overcome foot-related issues by regular...
