Introduction
Among many healthcare roles, nursing job is one of the integral parts of healt
hcare system, where the nurses are expected to create and maintain a proper therapeutic relationship with the patient and with the multi-disciplinary team involved with the care for the patient. In this process of nursing care, a person-centred care focus is necessary, which will enable the nurse to treat a patient individually, rather holistically. In this case, study, a details discussion will be made with respect to the application of person-centred care approach to treat the complex and chronic health conditions. Further, the presentation of the evidence based person-centred care will be presented, which will be framed by communication, resources integration and collaboration with the interdisciplinary healthcare team.
Person-Centred Approach in Care System
Many people suffer from chronic health diseases, which means they are suffering from illness for more than six months and they cannot be cured, and the presence of co-morbidities makes the case a complex one (Renzi et al., 2019). Ivy Knowles is a 93-year-old woman, who had retained injuries after a head on collision accident taken place at the speed of 60 kph. Her daughter was driving the vehicle and had suffered injuries. Ivy had a history of hypertension, hyperthyroidism, osteoporosis and cataract. She suffered chest injuries because of this accident, and from the preliminary investigation, it is predicted that she might have suffered rib fracture, which can eventually cause a lack of perfusion, lung collapse, internal bleeding and intense pain. In order to avoid any further development of severe problems, a high priority for the application of secondary prevention strategy is needed. Considering the early diagnosis and immediate treatment, a proper nursing care is required, which will focus on performing more airway, breathing, circulation, disability, exposure (ABCDE) assessment, having oxygen readily available and making sure that there are drug relief medicines charted for the patient based on the need. Kingston et al. (2017) have discussed that emergency nursing care in the blunt chest injuries requires a thorough assessment and a timely intervention, which includes the maximisation of the respiratory function and the reduction of the pain, which will be properly achieved by the oxygen therapy, adequate pain relief drugs and emergency basis chest physiotherapy. There are many activities, which requires inclusion in nursing tasks such as making an appointment for chest examination or any kind of imaging that the clinical requires and ensuring that the bloods are taken in the event that the hypovolemia occurs. Choi et al., (2018) have further elaborated that haemothorax should be always considered, whenever a rib fracture is detected and computed tomography also known as CT scan, should be used, as it is the most accurate way to identify the tiny amounts of pleural fluid.
The condition of Ivy is so much critical that she will need nurses, doctors and other members of healthcare team to assist her in being treated properly. To provide an efficient care system a range of health care professionals consisting of interdisciplinary team will be required for the extensive care of the patients.
