This assessment task requires you to watch a video of a conversation between Aged Services Emergency Team (ASET) Nurse, Robyn, and an older man, Max. This assessment task challenges you to use your critical thinking skills, knowledge of normal ageing changes, patterns of health, social interaction and illness in the older person. It requires you to take a holistic view of the older client and the effective ways to work with an older person to optimise health.


For this assignment we are looking to promote understanding of comprehensive aged care assessment including


(1)Pain and pain assessment in the older person;



(2) Poly pharmacy and medication review;


(3) Person centred care and


4) Health coaching.


Most importantly, we are looking for you to apply these concepts to the needs of an older man, Max. You will critique theASET Nurse's assessment of Maxas she prepares him for discharge from the Emergency Department of a rural hospital. Max is returning home after readmission for post-operative complications following a knee replacement.


The video conversation you will need to critically evaluate for this assessment task is found below in this section. Watch the video recording of the conversation with Maxat least onceand respond to the following questions using current evidence and references to support your critique and suggestions.




Question 1: Pain and Pain management in the older person






Max mentioned he had some pain during his post operative period. Discuss the impact pain can have on an older person and identify and discuss one evidenced based nursing related assessment tool suitable to use to identify the presence and intensity of pain in an older person. (Approx 400 words)

Part of the ASET Nurse's role is to ensure a person centred care approach is used when communicating with the older adult. Critique the ASET Nurse's use of effective (and/or active) listening in her conversation with Max. Include in your response the techniques used by the ASET Nurse to effectively collect the required information to assist in her decision making processes to ensure appropriate services were arranged for Max prior to his discharge. Question 4: Health Coaching


In the video on the Subject I2 site, Max reveals some of his health concerns such as depression, osteoarthritis, and medications. Evaluate the ASET Nurse’s use of the principles of health coaching; what extent is it effective in supporting Max to make changes to his own lifestyle? (Approx 400 Words).

Introduction
Post-operative care and pain management are the two most challenging aspects in the aged care sector. In fact, the dimensi
ons of care keep shifting from one end to the other, as per the changes in the conditions of the elderly people (Tong et al., 2018). As per the observation of the conversation of Robyn with Max in the provided video, there are multiple aspects of care, which have been discussed throughout.
Robyn’s approach is person centred and it gives some insightful views about how to ensure to deliver the perfect care in the setting preferably chosen by max. The evidence-based practices suggest having proper assessment of his pain and guiding him to manage the same. At the same time, it is the duty of the care assistant to help him recognise the best ways with regard to increase his social activities, giving him strength at psychological level.
Question One: Pain and Pain Management in Older Person
If seen from close perspective, pain is the outcome of some potential issue due to age factor in the context of people such as Max. He mentions about pain because of the fact that he has recently undergone through knee replacement surgery and in post-operative care; symptoms of pain are usual. In medical terms, pain is subjective and it has physiological and psychological impacts over aged individuals such as Max (Nawai et al., 2017).
The sensory response to pain enhances at this age due to lack of focus. Individuals having memory loss or partial dementia may constantly complain about having pain, which is comprehensive and recurring in their case (Lee et al., 2020).
Max, on the other hand, looks pensive and integrated while having conversation with Robyn, however, his pain management is one of the issues that need formal management. In order to make sure that his pain is managed through self-care, a formal assessment is deemed necessary—Robyn asks specific questions related to the pain management during the video.
Besides, it is quite a helpful procedure to the patients such as Max to identify the ways of managing their pain overall. Additionally, in order to evaluate the degree of pain in elderly, there are specific tools, which can be applied. Visual Analogue Scale or Categorical Rating Scale can be applied in the condition of pain.
These tow assessment tools are useful in assessing the pain and its intensity in older people, however, each of these tools need to be applied in particular contexts of care. VAS is applicable when the patient is not having any visual impairment. There is a 10cm scale, which is used during the assessment (Lee et al., 2020).
This scale is provided to the patient and or she is requested to mark the point over the scale that he or she precisely reckons defines the exactness of his or her pain. On the one end of the scale, “no pain” is mentioned while on the other, there is “severe pain” is marked. In case of visual impairment or dementia—a condition where patients are not able to figure out, which number they have to mark as they have lost their memory and recognition power—VAS is not recommended.
Categorical Rating Scale (CRS) is the assessment tool, in which particular words over the scale are applied (Morad & Farrokh, 2018). There can be many categories of pain, which individuals can mark on their own. They can be listed as mild, moderate, severe and critical.
Acute can also be a category to define such pains. CRS is mostly useful because it gives a particular intensity level of pain and helps the nursing professionals to verify the process of pain management, as per the identification of pain (Tong et al., 2018).
