Case Study Scenario: It is 0800 hours and you are working in a Paediatric Unit in a large metropolitan hospital. You are caring for Shaun Cross, a 15 -year-old male, who has Type I Diabetes. Shaun has been admitted for an exploratory arthroscope on his right knee, from an injury incurred whilst playing soccer. Shaun has fasted from 0600 hours and continues to fast (Nil by Mouth) in preparation for the surgery which is booked for 1000hours. Shaun last administered insulin to himself at 2000 hours the previous evening. Medical history: Nil known allergies. Shaun was diagnosed with Type I Diabetes six (6) months ago. Shaun’s diabetes has been well controlled with Lantus 50 units BD (mane and nocte). His last HbA1c three (3) weeks ago was 6.4% / 48mmmol. Introduction
    Shaun Cross is a 15-year-old patient, who has been suffering from type-1 diabetes. The patient has no major problem related to diabetes but he has been admitted to the hospital for an exploratory arthroscopy. He has injured himself while playing soccer
and now he is required to be operated on his right knee. According to the procedures of the operation, the patient needs to have fasted around 1000 hours. It is a mandatory factor for the patient to fast for 1000 hours. As the patient has diabetes, he has taken the insulin 2000 hours before. The pre-operative assessment of the regular check-up of the patient has shown some positive results. It shows that the patient is ready to be operated on within the allocated time. The blood pressure, heart rate, and respiratory functions are running well as per the requirement of the operation. Therefore, the patient is prepared to be operated on for his exploratory arthroscope.
Pre-Operative Care
    The management of traffic levels for patients who are about to be operated on requires serious and critical observation by the nurse. Especially for diabetic patients observe report to search as many factors need to be checked before the operation. The surgical stress creates different effects on the Glucose level that can be considered to maintain the optimum glycaemic control of the patient. Some special challenges can create target ranges for keeping up with the glucose level in the system of the patient (Pennington et al., 2020). It is always very hard to check on the different levels of bodily systems when it comes to treating people with hyperglycaemia. In this medical condition, the patient is diagnosed with Type 1 Diabetes, which is caused due to the destruction of the B cell that leads to insufficiency of insulin in the body.
Since this particular disease can cause harm to the insulin level in the body the glucose level of the patient is to be checked consecutively to maintain a normal level of blood glucose in the system. In order to provide necessary measurements, it is suggested for the nerves to provide food and water that are related to the medical condition of the patient. Since Shaun will be operated within the next thousand hours, we cannot be fed with food. Therefore, do not need to provide him with proper liquid food that can provide a balance between insulin and carbohydrates as per the requirement of the patient. Type 1 Diabetes patients can other their insulin insufficiency from carbohydrates and insulin and that can be generated from normal food.
The pre-operative stage needs to be observed with proper care since the axis of hyperglycaemia restricts the process of operation altogether. The general symptoms that can indicate hyperglycaemia or the increase of blood glucose level in the system of the patient include the increased thirst, frequent urination, fatigue, nausea and vomiting, dry mouth, stomach pain, shortness of breath, and fruity odour (Windmann et al., 2019). All these symptoms of improvement in blood glucose level can indicate serious problems especially when the patient is under pre-operative care. Judging by the observation on the patient it is indicated that a patient is not having any of the symptoms mentioned above and in the blood glucose level is normal which is 120/80. The blood pressure is 115/60 mm Hg. The condition of the patient in indicates that he is ready for the operation to be preceded properly.
