You are working in a general practice (GP) clinic as a registered nurse. Your next patient is Mr Alexander Kumar and his Wife Sarah. Alexander is 85 years of age and is the main carer for his wife who has dementia. Alex has not attended the clinic for himself in over 3 years, but normally attends with his wife for regular check-ups.



When Alex and Sarah arrive, you notice that Alex is limping and both Alex and Sarah look underweight. You also notice that both their clothes look a little dirty, which is unusual for them. When you read their medical file, you note that they have no services in place and their children all live interstate. When you ask Alex, what has brought him in, he tells you he has a wound on his leg which he has been dressing at home for three months, but it is not getting better. Alex tells you he hasn't seen a doctor about his leg earlier as he can no longer drive and finds it difficult to catch public transport with his wife, who sometimes becomes agitated and upset.

1. Aged care services and interdisciplinary care
Domiciliary (Home) Care is the type of home care services which is provided to the patients or elderly people in their own house. Through this care service the home caretaker or care assistant can be booked to visit the house of Alex and Sarah once or twice per day
to take care of them and do the following tasks on their behalf:
a. The caretaker can assist Alex and Sarah to get them dressed in the morning and then in the evening removes their clothes and helps them to get dressed with light night wears before going to the bed at night.
b. Then the caretaker must cook meals along with nutritious and healthy foods along with housework like cleaning the house and washing the dishes.
c. The caretaker must assist both Alex and Sarah to do shopping like buying foods, medicines, clothes so that it becomes easier for them to live a safe and secured life.
d. The home caretaker must also communicate; stimulate both Alex and Sarah with positive words and stories while delivering service. This will encourage both recovering fast, being happy and optimistic of their lives.
e. The caretakers must also help to clean their clothes, help them during dressing their wounds so that they are able to maintain a hygienic physical condition (Care for family, 2021)
Day-care centers are the places made for the old people so that they can visit once or twice a week and can spend time as well as avail various medical treatments necessary for curing their diseases. The day care center will be having own transport facilities which will pick-up the elderly people from their home and even drop them after medical treatment. Apart from these Alex and Sarah can avail various other opportunities from the Day care center as below:
a. In the day care center Alex and Sarah can spend the full day by themselves along with meeting other people of their age and communicate with them. They can also get themselves involved in some recreational activities like art and crafts, singing songs and nostalgic programs, exercises which can keep them mentally and physically fit and strong and bingo can be done at the day care centers.
b. They can also be provided with meals which are healthy and nutritious. Also they can take bath and shower in the day care center along with facility to roam in the garden.
c. The wounded leg of Alex can be dressed with the help of well trained and equipped nurses of the day care center.
d. Both Alex and Sarah are having co-morbidity; they must spend atleast two to three days in the day care facility so as to avail the health care facilities along with monitoring of their health. Also one of the staff members is a trained and experienced nurse and will be able to monitor the health (Australian Government, 2011).
Alex is a chain smoker and now due to this he is examined with various health related hazards like hypertension and weight loss. There have been small wounds which happened three months ago and now he has been prescribed to use pawpaw ointment and do dry dressing twice every week. But, the wound even after three months did not healed and in such circumstances it can lead to vascular abnormality. Therefore, it is recommended that Alex should follow the below steps...
